BACKGROUND INFORMATION

The Department of Public Health (DPH) currently operates Federally Qualified Health Centers (FQHC) located in Adelanto, Hesperia, Ontario, and San Bernardino. FQHCs are required to follow United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) regulations. For patient continuity of care, HRSA requires FQHCs enter into arrangements with hospitals to establish procedures for direct patient care, the exchange of information, emergency department visits, and the hospitalization of FQHC patients including hospital admission and discharge planning for patients receiving treatment at such hospitals.

The MOU with Montclair Hospital Medical Center will establish the HRSA required procedures for any DPH FQHC patient visiting the hospital for direct patient care, emergency department visits or hospitalization to that facility. The MOU will be effective for a five-year term and may be terminated by either party with or without cause upon 30 days written notice. DPH has had a non-financial MOU with Montclair Hospital Medical Center since 2020.
