FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Upon approval by the Board of Supervisors (Board), the Department of Public Health (DPH) will receive an increased award from the Medi-Cal Managed Care Plan (MCP) in the amount of $300,000 from Inland Empire Health Plan (IEHP). Adequate appropriation and revenue have been included in DPH’s 2026-27 recommended budget.
BACKGROUND INFORMATION
As part of the statewide California Advancing and Innovating Medi-Cal (CalAIM) initiative, the California Department of Healthcare Services (DHCS) requires all MCPs throughout California to collaborate with local health jurisdictions in the development and implementation of their Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The CHA and CHIP are required for DPH to maintain national accreditation with the Public Health Accreditation Board. DHCS requires MCPs to contribute resources in the form of direct funding, in-kind staffing and data to promote a comprehensive understanding of the multiple factors contributing to community health outcomes. MCPs have agreed to provide funding to DPH, subject to the availability of funding.
The funding from each MCP will allow DPH to increase foundational capabilities by hiring two Health Educator Specialist IIs, three Health Educator Specialist Is, and one Office Specialist to staff a variety of community engagement events and ensure quality improvement during the CHIP implementation process. MCP funding also covers the costs of a short-term consultant contract, selected through competitive procurement, to conduct stakeholder training and CHIP implementation evaluation and recommendations. Investment in these areas will drive lasting advances in public health capacity and systems to better serve and support those throughout San Bernardino county.
On June 10, 2025 (Item No. 87), the Board approved Agreement No. 25-413 (Agreement) with IEHP to establish a partnership with DPH for the development of the CHIP and implementation of the CHA, in the amount of $300,000, for the period of May 20, 2025 through June 30, 2026.
On June 23, 2026 (Item No. 112), the Board approved Amendment No. 1 to the Agreement with IEHP to continue collaborating on the CHIP and CHA initiatives, with no change to the amount of $300,000, and extending the term by four months, for the period of May 20, 2025 through November 1, 2026.
On September 2, 2026, DPH received Amendment No. 2 to the Agreement with IEHP. This amendment replaces the scope of services and the funding schedule, increases the Agreement amount by $300,000, from $300,000 to $600,000, corrects the effective date from May 20, 2025 to June 10, 2025, and extends the term by eight months, for a total agreement period of June 10, 2025 through June 30, 2027. The term extension aligns the Agreement’s expiration date with DPH’s other managed care plan agreements, including those with Molina Healthcare and Kaiser Permanente, which also expire on June 30, 2027.
Approval of this item will ensure continuity of activities supporting the CHIP and CHA and maintain the partnership between DPH and IEHP in meeting the DHCS requirements. The additional funding and extended term will support ongoing community engagement efforts and staffing resources necessary to advance implementation of the CHA toward established community health improvement goals.
