FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The cost of $800,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2024-25 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
On December 26, 2021, the Purchasing Agent approved SAP contract No. 4400023536 (SAP Contract) with Teleflex LLC (Teleflex) for the purchase of vascular access supplies for ARMC’s operating room. ARMC issued purchase orders up to its authority of $200,000 per year. However, due to an increase in the vascular access supplies needed for ARMC’s operating room, ARMC anticipated that the Purchasing Agent’s statutory authority would be exceeded, and requested approval by the Board of Supervisors (Board). On August 20, 2024 (Item No. 17), as the result of a non-competitive procurement, the Board authorized the Purchasing Agent to issue purchase orders with Teleflex for vascular access supplies in the not-to-exceed amount of $400,000, for the period of August 20, 2024 through November 30, 2026.
There continues to be a significant increase in the volume of patients requiring specialized vascular access, as identified by surgeons during surgeries. Therefore, there is a need for increased vascular access supplies. This item requests Board approval to authorize the Purchasing Agent to continue to issue purchase orders for the period of March 11, 2025 through November 30, 2026, in the not-to-exceed amount of $800,000 to meet this increased need.
Purchase orders allow ARMC to continue to purchase surgical supplies used for patients requiring specialized vascular access, which include peripherally inserted central catheter (PICC) lines and midlines from Teleflex. PICC lines are used for longer durations (weeks to months) while midlines are suitable for patients needing therapy for more than five days but less than four weeks. Patients with PICC lines and midlines, if medically stable, can continue treatment outside of the hospital setting, decreasing the length of stay. To continue to offer this service, the supplies are needed by the ARMC PICC line team to provide specialized vascular access to the community.
ARMC recommends approval of the purchase orders to allow ARMC’s operating room to continue to obtain supplies needed for patients requiring specialized vascular access to provide for the safety and health needs of county residents.
