Recommendation
Authorize the Purchasing Agent to issue purchase orders with Teleflex LLC for vascular access supplies in the aggregate not-to-exceed amount of $400,000, for the period of August 20, 2024 through November 30, 2026.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The cost of $400,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2024-25 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
On December 26, 2021, the Purchasing Agent approved SAP contract No. 4400023536 (“SAP Contract”) with Teleflex LLC (Teleflex) for the purchase of vascular access supplies for ARMC’s operating room. Thereafter, ARMC issued purchase orders up to its authority of $200,000 per year. However, due to an increase in the vascular access supplies needed for ARMC’s operating room, ARMC anticipates that the Purchasing Agent’s authority will be exceeded, and therefore request approval by the Board of Supervisors (Board) for the Purchasing Agent to continue to issue purchase orders in an aggregate not-to-exceed amount of $400,000 for the period of August 20, 2024 through November 30, 2026.
Surgical supplies used for patients requiring specialized vascular access include peripherally inserted central catheter (PICC) lines and midlines. PICC lines are used for longer durations (weeks to months) while midlines are suitable for patients needing therapy for more than five days but less than four weeks. Patients with PICC lines and midlines, if medically stable, can continue treatment outside of the hospital setting, decreasing the length of stay. To continue to offer this service, the supplies are needed by the ARMC PICC line team to provide specialized vascular access to the community. Over the past few months there has been a significant increase in volume in patients requiring specialized vascular access.
ARMC recommends approval of the purchase orders to allow ARMC’s operating room to continue to obtain supplies needed for patients requiring specialized vascular access and provide the safety, health and safety service needs of county residents.
PROCUREMENT
The Purchasing Department continues to support this non-competitive procurement based on functional specifications of Teleflex supplies as confirmed by the ARMC Value Analysis Committee. County Policy 11-04 requires departments to seek Board approval when contracting for non-competitive purchase of goods of more than $200,000 during a single annual period.
