FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The recommended contract amount of $2,555,000 for Enhanced Assisted Living program services is 100% funded by Mental Health Services Act funding. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2020-21 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
The Enhanced Assisted Living program provides unlocked Assisted Living Facilities that are licensed and governed by the State of California, Community Care Licensing Division, providing 24/7 services to adult and older adults over the age of 50 who require care and supervision as well as assistance with performing their Activities of Daily Living. Crest Home for The Elderly creates a safe and stable environment for adult and older adults who are high utilizers of acute medical and psychiatric care and affords the opportunity to divert placement into higher levels of care such as a Skilled Nursing Facility or a State Hospital.
Crest Home for The Elderly provides comprehensive medical care such as assistance in all Activities of Daily Living, monitoring vital signs, wound care, medication management, and injections to those consumers who also experience a comorbid behavioral health condition and require support and monitoring to ensure their psychiatric stability. In addition, Crest Home for The Elderly’s two facilities, located in Norco, provide organized therapeutic activities for recreation, social interaction, and memory enhancement that promotes quality of life and improves functionality in daily living and social skills, preparing the consumers to return to a less structured facility when appropriate. DBH anticipates that Crest Home for The Elderly will provide approximately 9,125 bed days for the term of the agreement, at a daily bed rate not to exceed $280 per bed per day.
PROCUREMENT
Due to the challenging nature of this population, placement of consumers who may need this level of residential setting can be difficult. In addition, there are consumers who have been hospitalized at our highest level of care (State Hospital) who may be ready for discharge to a least restrictive setting; however, due to their long term hospitalization, significant medical needs and behavioral management concerns, there are a limited number of assisted living facilities who are willing to partner with our department. DBH contacted several assisted living facilities in the area to inquire if there was an interest in working with this population; however, these facilities were not receptive to admitting consumers with high acuity behavioral health needs transitioning from State Hospitals. At this time, Crest Home for The Elderly is willing to provide Enhanced Assisted Living program services to this population, which assists in expanding the continuum of care for county residents.
The recommended non-competitive contract with Crest Home for The Elderly will allow DBH to provide Enhanced Assisted Living program services to consumers with behavioral conditions requiring this level of residential care. Purchasing concurs with this non-competitive contract with a justification that no other facility has been willing to accept clients who experience behavioral health conditions with co-occurring significant medical conditions. Contractor performance will be monitored by DBH on a regular basis to ensure compliance standards are met.
