RECOMMENDATION(S)

Recommendation

1.                     Accept and approve Amendment No. 2 to Agreement No. 23-833 with the California Department of Public Heath for the Future of Public Health funding to provide continued support for public health workforce and infrastructure, decreasing the amount by $3,652, from $33,323,578 to $33,319,926, with no change to the total grant performance period of July 1, 2023 through June 30, 2026.

2.                     Approve and authorize the submission of the one-year Acknowledgement of Allocation Letter, Annual Certification, and Annual Spend Plan, as required by the California Department of Public Health, to continue to receive Future of Public Health funding through June 30, 2026.

3.                     Authorize the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to execute and submit any subsequent non-substantive amendments and documents, including work plans and budgets required to secure Future of Public Health funding, to the California Department of Public Health related to Agreement No. 23-833, on behalf of the County, subject to review by County Counsel.

4.                     Direct the Director of the Department of Public Health to transmit all non-substantive amendments, in relation to Agreement No. 23-833 for the Future of Public Health funding to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

This item does not require additional Discretionary General Funding (Net County Cost). The allocation in the amount of $11,015,929 in 2025-26 from the California Department of Public Health (CDPH) for Future of Public Health (FoPH) funding does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2025-26 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

The CDPH provides $188,200,000 annually in FoPH funding to local health jurisdictions to support the public health workforce and infrastructure. CDPH requires that at least 70% of funds support the permanent public health workforce, and authorizes up to 30% for equipment, supplies, and other administrative costs such as facility space, furnishings, and travel. DPH verifies these requirements are met by tracking the use of FoPH funding through program codes in the Employee Management and Compensation System to fund positions within multiple DPH programs that historically would not be grant or fee-funded. In addition, DPH is required to submit an annual work plan and spend plan for review and consideration of the following fiscal year’s allocation.

 

On August 8, 2023 (Item No. 37), the Board of Supervisor (Board) approved Agreement No. 23-833 with CDPH FoPH funding, in the aggregate amount of $33,853,248 for the period of July 1, 2023 through June 30, 2026, which reflected a $11,284,416 annual allocation between 2023-24 and 2025-26.

 

On September 24, 2024 (Item No. 58), the Board approved Amendment No. 1 to Agreement No. 23-833, which involved a reduction of $529,670, decreasing the contract amount from $33,853,248 to $33,323,578, for the grant performance period of July 1, 2023 through June 30, 2026, to reflect DPH’s portion of a statewide FoPH annual allocation reduction effective 2024-25.

 

On July 2, 2025, DPH received an allocation notice from CDPH to Agreement No. 23-833, decreasing the anticipated 2025-26 funding by $3,652, from $11,019,581 to $11,015,929, thereby decreasing the total contract amount from $33,323,578 to $33,319,929, for the grant performance period of July 1, 2023 through June 30, 2026.

 

DPH will return to the Board to accept further FoPH funding from CDPH, if and when it is received. This item is being presented at this time as this is the first available date following the required operational, fiscal, and legal review. Approval at this time will not impact DPH services or activities. DPH is recommending delegated authority to execute and submit any non-substantive documents or amendments, including work plans and budgets, in relation to Agreement No. 23-833.

 

The Recommended actions will allow DPH to accept the FoPH funding allocation and submit the Acknowledgment of Allocation Letter and Annual Certification, along with the required supporting documentation, to CDPH by the extended deadline of August 29, 2025.
