RECOMMENDATION(S)
Recommendation
1. Approve Amendment No. 1 to Employment Contract No. 25-01 with Dr. Farah Hazim, D.D.S., to continue to provide dental services, revising the duties and responsibilities language, in the amount of $93.48 per hour, and extending the contract for two additional years, for a total period of January 25, 2025, through December 31, 2027, with the option to extend two additional one-year periods.
2. Authorize the Director of the Department of Public Health to execute amendments to extend the term of the Employment Contract with Farah Hazim, D.D.S. for a maximum of two additional successive one-year periods, on behalf of the County, subject to County Counsel review.
3. Direct the Director of the Department of Public Health to transmit any subsequent amendments to the Employment Contract with Farah Hazim, D.D.S. to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
This item will not result in the use of Discretionary General Funding (Net County Cost). The per hour cost of Employment Contract No. 25-01 (Contract) with Dr. Farah Hazim, D.D.S. (Dr. Hazim), at an initial rate of $84.27 per hour and a current rate of $93.48, will be funded by several funding sources, including, but not limited to, Medi-Cal, private insurance, private pay, and existing ongoing 1991 Realignment funds. Adequate appropriation and revenue have been included in the Department of Public Health (DPH) 2025-26 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
DPH provides comprehensive dental services to uninsured and underinsured populations throughout the county. An existing contract Public Health Dentist position provides routine preventive care and emergency dental services, including diagnosis and treatment plans, general restorative procedures, endodontics and minor oral surgery.
On January 14, 2025 (Item No.47), the Board of Supervisors approved an employment contract with Dr. Hazim for the contract position of Public Health Dentist, to provide dental services, at the initial rate of $84.27 per hour, for the period of January 25, 2025, through December 31, 2025.
Approval of Amendment No. 1 to Employment Contract No. 25-01 revises the contracted position duties and responsibilities to specify Dr. Hazim will provide dental services exclusively at two federally qualified health centers located in San Bernardino and Ontario, along with occasional coverage of mobile events. Approval of the amendment will also exercise the current option to extend the contract two additional years for the total contract period of January 25, 2025, through December 31, 2027, and add the option to extend the contract for two additional one-year periods.
