Recommendation

Approve Amendment No. 4, effective July 28, 2020, to Contract No. 15-218 with Orchid Court, Inc. for the provision of Enhanced Board and Care Services, increasing consumer access to beds, adding a second location, and increasing the total contract amount by $69,000, from $3,367,466 to $3,436,466, with no change to the total contract period of July 1, 2015 through September 30, 2020.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The recommended increase of $69,000 for Enhanced Board and Care services is funded by the Substance Abuse and Mental Health Services Administration Community Mental Health Services Block Grant and 1991 Realignment. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2020-21 budget.

 

BACKGROUND INFORMATION

Enhanced Care Services, within a state-licensed 24/7 residential facility, is a vital tool within the continuum of care for adult consumers with medical and behavioral health conditions in their journey towards independence. Providing an array of enhanced services, with 24/7 supervision and care, allows emotional support, stabilization, self-reliance, life skills improvements, and integration into the community. These services have enabled DBH to reduce a consumer’s length of stay in acute psychiatric hospitals, long-term locked psychiatric residential facilities, as well as aiding in the transition to a lower level of care while still having access to specialty services.

 

Orchid Court, Inc. (Orchid Court) has been providing state-licensed 24/7 residential care since 1995 and has collaborated with DBH since 2003 to provide enhanced care services to meet the specialized needs of the hard-to-place adults experiencing medical and behavioral health conditions. The requested increase in 2020-21 to the contract with Orchid Court will allow additional consumers to transition from a higher level of care that have been successful in learning self-care and are preparing to move to community housing. It also provides DBH access to a second site managed by Orchid Court. These consumers are close to successfully maintaining stability in the community, but still need the added supervision, services, structure, and support provided by Orchid Court, which is a vital component of the DBH continuum of care.

 

The Enhanced Care Services provided by Orchid Court include, but are not limited to: diabetic services, transportation to and from medical and psychiatric appointments, life skills education and activities, daily on-site treatment groups and educational classes, substance use disorder education and treatment, and services for the hearing impaired. Additional specialty services are provided to consumers experiencing serious medical and behavioral health conditions and whose behavior continues to negatively impact the ability to find placement in a lower level of care. DBH anticipates with the additional funds that Orchid Court will provide an additional 950 bed days for a total of 3,650 bed days, at a daily bed rate not to exceed $75 per bed, per day, during the final quarter of the contract term.

 

PROCUREMENT

On May 5, 2015 (Item No. 18), as the result of a formal procurement, the Board of Supervisors (Board) approved the contract with Orchid Court for the provision of Enhanced Care Services, in the amount of $1,474,080, for up to 30 DBH consumers per day at $40 per bed day, for the period of July 1, 2015 through June 30, 2018, with two one-year extension options contingent upon funding and contractor performance. 

 

On April 17, 2018 (Item No. 13), the Board approved Amendment No. 1 updating standard contract language, increasing the daily bed rate to $55, exercising the first one-year option to extend the contract, and increasing the total contract amount by $757,623, from $1,474,080 to $2,231,703, for the total contract period of July 1, 2015 through June 30, 2019.

 

On March 12, 2019 (Item No. 19), the Board approved Amendment No. 2 updating standard contract language, exercising the final one-year option to extend the contract, and increasing the total contract amount by $862,013 from $2,231,703 to $3,093,716, for the total contract period of July 1, 2015 through September 30, 2020.

 

On March 10, 2020 (Item No. 23), the Board approved Amendment No. 3, increasing the daily bed rate to $75 and increasing the total contract amount by $273,750, from $3,093,716 to $3,367,466, with no change to the total contract period of July 1, 2015 through September 30, 2020. 

 

DBH will continue to monitor contractor performance on a regular basis to ensure compliance standards are met.
