Recommendation

Approve contracts with the following agencies, for the provision of Enhanced Assisted Living services, in the aggregate amount of $5,112,800, for the period of July 1, 2025, through June 30, 2030:

1.                     Crest Home for the Elderly.

2.                     GHC Upland RCFE, LLC dba Heritage Court Assisted Living.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The recommended total aggregate amount of $5,112,800 for the recommended contracts for Enhanced Assisted Living (EAL) services is funded by Mental Health Services Act revenue. Adequate appropriation and revenue will be included in the Department of Behavioral Health’s (DBH) future recommended budgets.

 

BACKGROUND INFORMATION

DBH is responsible for providing mental health and substance use disorder (SUD) services to county residents experiencing severe mental illness and/or SUD. EAL facilities provide comprehensive medical care to clients experiencing a co-morbid behavioral health condition and requiring support and monitoring to ensure their psychiatric stability. Facilities are licensed and governed by the State of California, Community Care Licensing Division.

 

EAL facilities create a safe and stable environment for adults and older adults who are high utilizers of acute medical and psychiatric care, allowing placement to be diverted away from higher levels of care such as Skilled Nursing Facilities or State Hospitals. Crest Home for the Elderly (Crest Home) and GHC Upland RCFE, LLC dba Heritage Court Assisted Living (GHC) will provide comprehensive medical care such as assisting with activities of daily life, monitoring vital signs, caring of wounds, and administration of medication to clients. DBH EAL will serve an estimate of 10 clients annually at an estimated cost of $102,200 per client.

 

DBH will 1) implement monitoring mechanisms to regularly review client service data and progress, 2) conduct site visits and annual monitoring to ensure performance and compliance standards of the contract(s), and 3) audit invoices administratively and programmatically to ensure client and/or bed counts are accurate prior to payment processing.

 

PROCUREMENT

Purchasing supports the non-competitive procurement of the EAL services based on specialized credentials. Due to the challenging nature of the population of older adults who are high utilizers of acute medical and psychiatric care, placement of clients who need this level of care can be difficult. There are a limited number of EAL facilities who are willing to partner with DBH due to the significant medical needs and behavioral management concerns of these clients. The recommended contracts with Crest Home and GHC will allow DBH to provide EAL services to clients requiring this level of care, which assists in expanding the continuum of care for county residents.
