RECOMMENDATION(S)

Recommendation

1.                     Approve and authorize the submission of the 2025 Non-Competing Continuation and Annual Progress Report, for continued funding from the Strengthening Public Health Infrastructure Grant Program, in the amount of $964,952, for the period of December 1, 2025 through November 30, 2026.

2.                     Authorize the Director of the Department of Public Health as the Authorized Organizational Representative to electronically execute and submit the 2025 Non-Competing Continuation and Annual Progress Report, and any non-substantive amendments necessary to secure funding, on behalf of the County, subject to review by County Counsel.

3.                     Direct the Director of the Department of Public Health to transmit the executed 2025 Non-Competing Continuation and Annual Progress Report, and any non-substantive amendments, to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

This item will not result in the use of Discretionary General Funding (Net County Cost). Submission of the 2025 Non-Competing Continuation (NCC) and Annual Progress Report (APR), for the amount of $964,952, for the period of December 1, 2025 through November 30, 2026, to the Centers for Disease Control and Prevention (CDC), does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2025-26 budget and will be included in the 2026-27 recommended budget.

 

BACKGROUND INFORMATION

The COVID-19 pandemic demonstrated the critical importance of creating a robust public health system to not only serve immediate needs, but to prepare for future public health emergencies. The CDC provides Strengthening Public Health Infrastructure Grant (Grant) funding to support core infrastructure improvements to public health agencies’ workforce (Component A1) and foundational capabilities (Component A2).

 

On August 9, 2022 (Item No. 34), the Board of Supervisors (Board) approved and authorized the submission of a Grant application to the CDC, in the amount of $33,018,272, for the period of December 1, 2022 through November 30, 2027.

 

On February 28, 2023 (Item No. 43), the Board approved Contract No. 23-153 (Federal Award No. 1 NE11OE000070-01-00) from the CDC for Grant funding, in the amount of $20,715,347, for the period of December 1, 2022 through November 30, 2023. The award included $19,750,395 in Component A1 funding, and $964,952 in Component A2 funding. The total Grant performance period is December 1, 2022 through November 30, 2027, with funding for subsequent years being based on satisfactory programmatic progress and the availability of funds.

 

On July 11, 2023 (Item No. 31), the Board approved amendment No. 1 to Contract No. 23-153 (Federal Award No. 6 NE11OE000070-01-01) from the CDC reallocating funds between budget line items, with no change to the original award amount of $20,715,347 or the total performance period of December 1, 2022 through November 30, 2027.

 

On July 27, 2023, the CDC released instructions for the 2023 NCC and APR for continued Grant Component A2 funding. On September 26, 2023 (Item No. 52) the Board approved and authorized the submission of the 2023 NCC and APR, in the amount of $1,688,651, for the period of December 1, 2023 through November 30, 2024.

 

On February 6, 2024 (Item No. 39), the Board approved Contract 24-132 (Federal Award No. 5 NE11OE000070 02-00) for continued Grant Component A2 funding, in the amount of $1,688,651, for the period of December 1, 2023 through November 30, 2024.

 

On July 15, 2024, using the authority delegated by the Board on July 11, 2023 (Item No. 31), DPH submitted the 2024 NCC and APR for continued Grant Component A2 funding, in the amount of $1,688,651, for the period of December 1, 2023 through November 30, 2024. At the time, the NCC submission did not require legally binding documents.

 

On March 11, 2025 (Item No. 34), the Board approved Contract No. 25-145 (Federal Award No. NE11OE000070-03-00) for continued Grant Component A2 funding, in the amount of $1,688,651, for the period of December 1, 2024 through November 30, 2025.

 

On June 26, 2025, the CDC released the 2025 NCC and APR for continued Grant Component A2 funding. The 2025 NCC and APR include documents which are legally binding and require Board approval. These documents were not present with the previous NCC and APR submission upon receipt of a Notice of Award. DPH will return to the Board for acceptance of the additional funding.

 

Approval of this item would allow DPH to receive additional Component A2 funding. The funding will be used to continue to improve San Bernardino County’s public health infrastructure by supporting the department’s performance management and accreditation efforts, development of policies and standard practices, updates to the electronic health record system, and improvement of financial management and procurement processes. The Director of DPH is the assigned Authorized Officer within CDC, as requested in Recommendation No. 2, requesting the Director of DPH to sign and submit the required documents.
