BACKGROUND INFORMATION
Amendment No. 4 (Amendment) to Agreement No. 20-1115 (Agreement) will add preventative maintenance service for additional ultrasound equipment at ARMC’s main campus. Services provided by Avante Heath Solutions (Avante) include two scheduled preventative maintenance inspections per year and technical phone support, 24 hours a day, 7 days a week, including weekends and holidays.
On November 17, 2020 (Item No. 22), the Board of Supervisors (Board) approved the Agreement with Avante, as a result of a competitive procurement, in the amount of $329,000, for the preventative maintenance of ultrasound equipment, for the term of December 5, 2020 through December 4, 2025.
On May 4, 2021 (Item No. 9), the Board approved Amendment No. 1 to the Agreement to increase the contract amount by $155,190, from $329,000 to $484,190, to add additional equipment for preventative maintenance, with no change to the Contract term.
On August 23, 2022 (Item No. 5), the Board approved Amendment No. 2 to the Agreement for preventative maintenance of 33 ultrasound probes, increasing the contract amount by $92,143, from $484,190 to $576,333, with no change to the Contract term.
