RECOMMENDATION(S)
Recommendation
1. Authorize the purchase of unbudgeted fixed assets, in the total amount of $450,000, for the following items:
a. Six Hemosphere Advanced Monitoring systems in the total amount of $420,000
b. One Simulation Manikin System in the amount of $30,000
2. Authorize the Auditor-Controller/Treasurer/Tax Collector to post budget adjustments in the amount of $450,000 as detailed in the Financial Impact section below to the Arrowhead Regional Medical Center for expenses related to the implementation of a Cardiovascular Program. (Four Votes Required).

FINANCIAL IMPACT
Approval of the item will not result in the use of Discretionary General Funding (Net County Cost). The $450,000 are funded by State Medi-Cal, Federal Medicare, private insurances and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act.
Arrowhead Regional Medical Center (ARMC) is requesting a budget adjustment in the amount of $450,000 to cover expenditures for equipment related to the implementation of a Cardiovascular Program. The costs in this request specifically pertain to six Hemosphere Advanced Monitoring systems at a total cost of $420,000 and one Simulation Manikin System at a total cost of $30,000. The original expenses were budgeted in the wrong SAP General Ledger Account. This item seeks to correct the accounts with a net budget adjustment of zero dollars. This item authorizes the Auditor-Controller/Treasurer/Tax Collector to post the following adjustments to the 2020-21 ARMC budget:

BACKGROUND INFORMATION
ARMC is a state-of-the-art, acute care facility. ARMC offers the latest in patient care by providing a full range of inpatient and outpatient services, four off campus community health centers, behavioral health inpatient services and numerous specialty services. ARMC is fully accredited by the Centers for Medicare and Medicaid Services (CMS) and the Joint Commission (TJC).
Approval of the purchase of the unbudgeted fixes assets will allow ARMC to purchase six Hemosphere Systems which allows the clinician to obtain pertinent blood pressure data necessary to guide the proper intraoperative and postoperative heart surgery patients' management. It provides past trends, current data, and predictive decision support in easy to use touch displays. This unit would monitor each patient during the postoperative recovery stage. Approval will also allow ARMC to purchase one Simulation Manikin System that will be used in the cardiac surgery post-operative recovery training of the ICU nurses. It provides the clinical educator the ability to simulate realistic clinical scenarios and provide a hands-on experience to the nurses; this helps augment the concepts learned during the teaching portion of the training.
ARMC has identified the need for and taken steps to fund a Cardiovascular Program, which is a new service line to provide coronary artery bypass grafting and valve replacements. These mid-year budget adjustments are needed to ensure ARMC is appropriately equipped with medical equipment to treat any potential San Bernardino County patients who require specialized Cardiovascular care.
Approval of this item will provide for the health and safety of County residents by ensuring the readiness of ARMC staff and facilities to treat any potential patients facing a Cardiovascular health crisis.
