RECOMMENDATION(S)

Recommendation

1.                     Approve Amendments, effective March 15, 2022, to the following contracts for Full Service Partnership services, increasing the total contract amount by $5,062,500, from $24,769,347 to $29,831,847, with no change to the contract period of July 1, 2018 through June 30, 2023:

a.                     Mental Health Systems, Inc., Amendment No. 4 to Contract No. 18-389, increasing the total contract amount by $3,712,500, from $10,204,357 to $13,916,857.

b.                     Valley Star Behavioral Health, Inc., Amendment No. 2 to Contract No. 18-390, increasing the total contract amount by $1,350,000, from $14,564,990 to $15,914,990.

2.                     Approve Amendment No. 1, effective March 15, 2022, to Contract No. 21-439 with Step Up on Second, Inc. for Full Service Partnership services, increasing the total contract amount by $12,082,500, from $30,442,150 to $42,524,650, with no change to the contract period of July 1, 2021 through June 30, 2026.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost).  The increase in the amount of $17,145,000 for Full Service Partnership (FSP) services is funded by Medi-Cal Federal Financial Participation and Mental Health Services Act funding.  Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2021-22 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

FSP services are designed to address chronic behavioral health conditions by providing a full spectrum of community and mental health services to support a client’s recovery, wellness, and resilience. There is an increased demand for permanent supportive housing due to numerous new housing development projects, which require an increase in the contract amounts for FSP program services.   The recommended contract amendments with Mental Health Systems, Inc. (MHS), Valley Star Behavioral Health, Inc. (Valley Star), and Step Up on Second, Inc. (Step Up) will allow DBH to provide an expansion of FSP program services to support eligible clients in obtaining and maintaining permanent supportive housing. DBH anticipates that MHS, Valley Star, and Step Up will provide additional FSP services to approximately 404 clients annually, at an average cost of $15,000 per client through this expansion of services.

 

The FSP services are a collaborative relationship between the contractor and client, and when appropriate, the client’s family.  Through this collaborative effort, the contractor plans and provides the full spectrum of community services (behavioral health and non-behavioral health services) for the client to achieve identified goals.  FSP services provide crisis intervention/stabilization, family education, peer support, and treatment programs that treat both substance use disorders and behavioral health conditions in an integrated fashion that include 24/7 access to an inter-disciplinary team of behavioral health professional staff and peer advocates.

 

FSP services also support community integration, such as food, clothing, housewares, shelter, employment education, and more using an open format that allows “Whatever it Takes” to help the client reintegrate into their community.  The goal of the program is to help reduce the need for emergency services for behavioral health concerns, homelessness, and criminal justice involvement for clients.

 

PROCUREMENT

On June 26, 2018 (Item No. 22), as the result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 18-389 with MHS for FSP services, in the amount of $9,632,155 and Contract No. 18-390 with Valley Star for FSP and Mental Health Court services, in the amount of $12,324,990, both for the period of July 1, 2018 through June 30, 2023.

 

On May 21, 2019 (Item No. 42), the Board approved Amendment No. 1 to Contract No. 18-389 with MHS increasing the total contract amount by $183,448 to $9,815,603 and adding two Housing Coordinator positions, with no change to the contract period of July 1, 2018 through June 30, 2023.

 

On October 27, 2020 (Item No. 22), the Board approved Amendment No. 2 to Contract No. 18-389 with MHS increasing the total contract amount by $176,707 to $9,992,310 and adding two Master’s level clinical case manager positions under Project Roomkey for the service period of September 1, 2020 through June 30, 2021, with no change to the contract period of July 1, 2018 through June 30, 2023.

 

On June 8, 2021 (Item No. 27), the Board approved the non-competitive Contract No. 21-439 with Step Up to provide FSP services to chronically homeless individuals, in the amount of $30,442,150, for the period of July 1, 2021 through June 30, 2026. Step Up is the only known provider in San Bernardino County that has demonstrated this particular experience and specialty dealing with individuals who are both chronically homeless and suffering with mental illness. Purchasing supports the non-competitive contract amendment with Step Up to provide additional FSP program services to these clients to reduce behavioral health disabilities and maintain stabilized housing by providing a full spectrum of community and behavioral health services to support recovery, wellness, and resilience.

 

On June 8, 2021 (Item No. 29), the Board approved Amendment No. 3 to Contract No. 18-389 with MHS increasing the total contract amount by $212,047 to $10,204,357, keeping the two Master’s level clinical case manager positions under Project Roomkey, for the service period of September 1, 2020 through June 30, 2022, with no change to the contract period of July 1, 2018 through June 30, 2023.

 

On October 26, 2021 (Item No. 22), the Board approved Amendment No. 1 to Contract No. 18-390 with Valley Star increasing the total contract amount by $2,240,000 to $14,564,990 and adding General Mental Health outpatient services and California Work Opportunity and Responsibility to Kids mental health services, with no change to the contract period of July 1, 2018 through June 30, 2023. DBH anticipates conducting a formal procurement for FSP and Mental Health Court services beginning in the fall of 2022, with contracts to be approved for a July 1, 2023 start date. 

 

DBH will continue to monitor contractor performance on a regular basis to ensure compliance standards are met.  Contract monitoring is inclusive of, but not limited to, site visits and assessments of policy, procedures, process implementation, and service delivery components to ensure compliance with contract and applicable laws and regulations.  The fiscal review ensures costs claimed are reasonable, allowable and in compliance with related financial records and procedures.  Additionally, as needed and appropriate, a review of Medi-Cal certification, related standards, and chart documentation may be completed.
