Recommendation
Approve amendments to the following contracts to provide medical care and support services under the Ryan White HIV/AIDS Part A HIV Emergency Relief Project Grants Program, increasing the total aggregate amount by $193,211 from $38,158,381 to $38,351,592, and revising the scope of work and budgets, with no change to the contract periods of March 1, 2021 through February 28, 2026:
1. Amendment No. 5 to Contract No. 20-1178 with AIDS Healthcare Foundation, increasing the contract by $16,593 from $3,209,963 to $3,226,556.
2. Amendment No. 5 to Contract No. 20-1180 with County of Riverside, Department of Public Health, increasing the contract by $21,752 from $4,116,062 to $4,137,814.
3. Amendment No. 5 to Contact No. 20-1181 with DAP Health, increasing the contract by $78,732 from $13,732,550 to $13,811,282.
4. Amendment No. 5 to Contract No. 20-1182 with Foothill AIDS Project, increasing the contract by $33,036 from $12,225,798 to $12,258,834.
5. Amendment No.6 to Contract No. 20-1183 with SAC Health System, increasing the contract by $33,472 from $3,138,022 to $3,171,494.
6. Amendment No. 5 to Contract No. 20-1184 with Young Scholars for Academic Empowerment dba TruEvolution, Inc., increasing the contract by $9,626 from $1,735,986 to $1,745,612.

FINANCIAL IMPACT
This item will not impact Discretionary General Funding (Net County Cost). The amendments increase the total aggregate contract amounts by $193,211, from $38,158,381 to $38,351,592. These increases are fully funded through reimbursements from the United States Department of Health and Human Services, Health Resources and Services Administration’s (HRSA) Ryan White HIV/AIDS Part A HIV Emergency Relief Grant (Ryan White Part A) Program. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2025-26 budget.
BACKGROUND INFORMATION
DPH has received funding from HRSA for the Ryan White Part A Program since 1994. Acting as the lead agency for the Transitional Grant Area of San Bernardino and Riverside counties, DPH oversees and administers the grant funds and assures compliance with grant requirements. DPH awards funding for the Ryan White Part A Program for to contractors to provide a comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with Human Immunodeficiency Virus (HIV) and their families. The services include mental health services, dental care, substance abuse treatment, medical nutrition therapy, early intervention services, case management, transportation, psychosocial support, emergency financial assistance, housing, and food assistance.
On December 15, 2020 (Item No. 55), as a result of a formal procurement, the Board of Supervisors (Board) approved contracts with the agencies listed in the Recommendation and Borrego Community Health Foundation (Contract No. 20-1179), in the total contract amount of $21,487,377, for the period of March 1, 2021 through February 29, 2024.
On July 27, 2021 (Item No. 36), the Board approved Amendment No. 1 to Contract Nos. 20-1178, 20-1179, 20-1180, 20-1181, 20-1182, 20-1183, and 20-1184 with the agencies listed in the Recommendation, decreasing the total contract amount by $750,948, from $21,487,377 to $20,736,429, for the contract period of March 1, 2021 through February 29, 2024. The decrease was due to a reduction in funding by HRSA.
On November 16, 2021 (Item No. 42), the Board approved Amendment No. 2 to Contract No. 20-1179 with Borrego Community Health Foundation and Contract No. 20-1183 with SAC Health System, adjusting the budget and scope of work with no change to the total contract amount of $2,459,340 for the contract period of March 1, 2021 through February 29, 2024. Borrego Community Health Foundation’s contract, in the amount of $1,359,629, was mutually terminated on March 24, 2022.
On December 6, 2022 (Item No. 35), the Board approved Amendment No. 3 to Contract No. 20-1183 with SAC Health System and Amendment No. 2 to Contract Nos. 20-1178, 20-1180, 20-1181, 20-1182, and 20-1184 with the agencies listed in the Recommendation, increasing the total amount of the contracts by $2,568,241 from $19,376,800 to $21,945,041, revising the scope of work and budgets, with no change to the contract period of March 1, 2021 through February 29, 2024. The increase in the amount of funding to the contractors was due to additional funding received from HRSA.
On September 26, 2023 (Item No. 53), the Board approved Amendment No. 4 Contract No. 20-1183 with SAC Health System and Amendment No. 3 to Contract Nos. 20-1178, 20-1180, 20-1181, 20-1182, and 20-1184 with the rest of the agencies listed in the Recommendation, increasing the total amount of the contracts by $16,069,052 from $21,945,041 to $38,014,093, revising the scope of work and budgets, and extending the contract for two years, for the total period of March 1, 2021 through February 28, 2026.
On November 19, 2024 (Item No. 38), the Board approved Amendment No. 5 to Contract No. 20-1183 with SAC Health System and Amendment No. 4 to Contract Nos. 20-1178, 20-1180, 20-1181, 20-1182, and 20-1184 with the rest of the agencies listed in the Recommendation, increasing the total amount of the contracts by $144,288 from $38,014,093 to $38,158,381, revising the scope of work and budgets, with no change to the contract period of March 1, 2021 through February 28, 2026. The increase in the amount of funding to the contractors was due to additional funding received from HRSA.
Approval of the Recommendation will increase the total amount of the contracts by $193,211, due to the final award received from HRSA and approved by the Board on October 7, 2025 (Item No. 29). The recommended amendments include revised budgets and scopes of work for each contract to reflect the increase in the number of visits and clients served.
