Recommendation
Approve Amendment No. 2 to Contract No.18-248 with Gregory A. Rex, D.V.M., dba Animal Medical Hospital Association II for veterinary services to animals at the Devore and Big Bear City animal shelters, increasing the amount by $175,000, from $600,000 to a total amount not to exceed $775,000, and exercising the second one-year extension option, for a total contract period of July 1, 2018 through June 30, 2023.

FINANCIAL IMPACT
This item will not result in the use of additional Discretionary General Funding (Net County Cost). The increase of $175,000 to the contract on a fee-for-service basis for veterinary services is funded by Discretionary General Funding, contract revenue and fees collected by the Department of Public Health (DPH) Animal Care and Control (ACC). Adequate appropriation will be included in DPH’s 2022-23 recommended budget.
BACKGROUND INFORMATION
The recommended amendment to the contract with Gregory A. Rex, D.V.M., dba Animal Medical Hospital Association II (Dr. Rex) enables DPH-ACC to retain a licensed veterinarian who provides services including the examination, treatment, and diagnosis of sick, injured, or diseased small animals; spaying or neutering dogs and cats which have been adopted; providing State-mandated veterinary services to stray and homeless animals at the Devore and Big Bear City animal shelters. The veterinarian will also treat and care for shelter animals at his facility when animals require treatment. The recommended amendment will execute the second one-year extension option, extending services through June 30, 2023.
Under the terms of the recommended amendment, DPH-ACC will have the ability to order medications and other items needed for the care and treatment of animals at the Devore and Big Bear City shelters through Dr. Rex at his cost. This will ensure the County is paying direct costs for medications obtained from wholesalers and thus avoiding additional fees or charges incurred in the acquisition of these necessary items.
Dr. Rex’s performance will be monitored by review of monthly reports and an annual site visit to ensure compliance with the administrative, program, and fiscal terms and conditions of the contract. Dr. Rex will cooperate with the County in the implementation, monitoring and evaluation of this contract and comply with any and all reporting requirements established by this contract. Dr. Rex’s charges must be for services provided to eligible beneficiaries under this funded program. Human Services Auditing will review the invoice and attachments remitted by Dr. Rex prior to payment. The County reserves the right to request additional program and/or fiscal reports to facilitate this monthly review. The contract may be terminated without cause by the County with 30 days written notice.
PROCUREMENT
On May 22, 2018 (Item No. 89), the Board of Supervisors (Board) approved Contract No. 18-248 with Dr. Rex to provide veterinary services to animals at the Devore and Big Bear City animal shelters, in an amount not to exceed $450,000, for the period July 1, 2018 through June 30, 2021, with two one-year extension options by mutual agreement by both parties.
On May 18, 2021 (Item No. 47), the Board approved Amendment No. 1 to Contract No. 18-248 with Dr. Rex for continued provision of veterinary services to animals at the Devore and Big Bear City animal shelters, increasing the amount by $150,000, from $450,000 to a total amount not to exceed $600,000, and exercising the first of two one-year extension options, for a total contract period of July 1, 2018 through June 30, 2022.
