RECOMMENDATION(S)
Recommendation
1. Approve Contract with Aegis Treatment Centers, LLC, for the provision of medications to treat individuals incarcerated in County jails who suffer from opioid use disorder, in an aggregate amount not to exceed $1,500,000, for the period of December 16, 2025 through December 15, 2028, with the option to extend the term by one additional two-year period, or two additional one-year periods.
2. Authorize the Purchasing Agent, as requested by the Sheriff/Coroner/Public Administrator or Undersheriff, to execute change orders to the contract, as needed, subject to review by County Counsel, as long as the aggregate amount of such change orders does not exceed $50,000 of the total contract amount, and the change orders do not amend the contract term.
3. Direct the Sheriff/Coroner/Public Administrator or Undersheriff to transmit all change orders to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The contract will be funded by the Opioid Settlement Funding as approved by the Board of Supervisors (Board) on August 19, 2025 (Item No. 26). A budget adjustment is not requested at this time but will be included in a future budget report.
BACKGROUND INFORMATION
The Opioid Use Disorder (OUD) continues to be a significant public health and safety concern, both within the community and in correctional settings. The absence of services to treat OUD increases risks of withdrawal complications for individuals during custody and substantially raises the likelihood of overdoses and deaths following release.
Currently, the County Jail Health Services has a formal medication-assisted treatment (MAT) program for OUD that dispenses the medication suboxone. However, in some cases, it may be medically advisable to treat an individual with methadone, which is an alternative drug approved by the Food and Drug Administration to treat OUD.
Approval of this Item will allow the MAT program within the County jail to align with state and federal best practices and correctional healthcare standards. Implementing this service will also enhance continuity of care by allowing individuals to begin or continue treatment while in custody and connect to community-based programs upon release.
Authorization for the Purchasing Agent to execute change orders to the contract will allow the Sheriff/Coroner/Public Administrator to address unforeseen fluctuations in the quantity of medication needed to provide treatment to those individuals in the MAT program.
PROCUREMENT
The California Department of Health Care Services (DHCS) is responsible for Narcotic Treatment Programs (NTP) oversight and licensing requirements; Aegis Treatment Centers, LLC is one of the two companies within the County that DHCS licenses to provide the required NTP services; therefore, a procurement was not conducted to provide the MAT for OUD. Purchasing supports the non-competitive justification based on Aegis' specialized credentials required to provide these services.
County Policy 11-04, Procurement of Goods and Supplies, Equipment and Services, requires departments to seek Board approval for non-competitive purchases of goods in excess of $200,000 annually.
