Approve Amendment No. 1 to Agreement No. 16-800 with Baxter Healthcare Corporation to increase the amount by $78,844 from $53,315 to $132,159 and to extend the contract for an additional five years for the total period of November 1, 2016 through November 1, 2024 to continue leasing the Exactamix Compounder Equipment and Abacus Software License for use in Arrowhead Regional Medical Center - Pharmacy Department.

FINANCIAL IMPACT
Approval of the recommendations will not result in the use of Discretionary Funding (Net County Cost), as the additional cost of $78,844 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue is included in the Arrowhead Regional Medical Center’s (ARMC) 2019-20 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
Approval of Recommendations will allow ARMC to continue to use the Exactamix Compounder and parenteral nutrition compounder software (ABACUS), which interfaces with Microsoft Windows® XP and Microsoft Windows® 10. The Exactamix compounder and ABACUS software enable the Pharmacy to compound patient specific neonatal and adult parenteral solutions safely and efficiently. The system automates complex calculations and provides clinically important ingredient compatibility and dose warnings to the pharmacist. This multi-ingredient compounder provides for the safety, health and social services needs of County residents by improving accuracy and safety by eliminating error prone manual calculations and preventing ingredient mix-ups.
The Exactamix compounder is the unit that actually mixes parenteral ingredients to formulate intravenous nutrition solutions that are tailored to the individual patient. The ABACUS software, which is designed to work only with the Exactamix compounder, is needed to perform complex formulation calculations, and provide compatibility/dose warnings.
PROCUREMENT
On October 18, 2016 (Item No. 9), the Board of Supervisors approved Agreement No. 16-800 with Baxter Healthcare Corporation for rental of the Exactamix Compounder and ABACUS software license in the amount of $53,315 for the three-year period of October 18, 2016, through October 17, 2019, with a two-year option to extend. The final agreement set the contract period at the later date of October 18, 2016 or the first day of the month following the signed acceptance of the Order Form, and did not include an option to extend the terms for two years.
This continued procurement is supported by a non-competitive justification based on equipment compatibility at ARMC. The Exactamix compounder has been the only automated adult/pediatric parenteral nutrition compounder in use at ARMC for over eight years and is only available through Baxter Healthcare Corporation (Baxter). Recently, B Braun developed a new compounder that has similar capabilities to Baxter's system. Due to eight years of experience with the Exactamix compounder at ARMC, especially in the pediatric population, the Baxter system is the preferred compounder at this time and guarantees no disruption to patient care. Purchasing concurs with the non-competitive justification of functionality.
