RECOMMENDATION(S)
Recommendation
1. Accept grant award from the California Department of Health Care Services for the Providing Access and Transforming Health - Capacity and Infrastructure Transition, Expansion and Development Program, Round 3, in an amount up to $352,638, for the performance period of August 20, 2024 through August 19, 2026.
2. Authorize the County Chief Executive Officer or ARMC Chief Executive Officer to approve, electronically execute, and submit acknowledgment of the Providing Access and Transforming Health - Capacity and Infrastructure Transition, Expansion and Development Program, Round 3, grant terms and conditions associated with the acceptance of the grant funds and administrative documents, including progress reports required under the grant, on behalf of the County, subject to review by County Counsel.
3. Direct the ARMC Chief Executive Officer to transmit the grant award documents, including the grant terms and conditions, to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Upon acceptance, the County will receive a grant in an amount not to exceed $352,638 for use by Arrowhead Regional Medical Center (ARMC), the Department of Behavioral Health (DBH), and the Department of Public Health (DPH) in fulfilling the scope of work specified by the grant terms. Adequate appropriation and revenue have been included in each department’s 2024-25 budgets and will be included in future recommended budgets.
BACKGROUND INFORMATION
California State Assembly Bill 133 (Chapter 143, Statutes 2021) authorized the Providing Access and Transforming Health (PATH) - Capacity and Infrastructure Transition, Expansion and Development (CITED) funding to support the implementation of the California Advancing and Innovating Medi-Cal initiatives, which includes the Enhanced Care Management (ECM) program. PATH CITED Round 3 provides funding to build capacity and infrastructure for the delivery of Medi-Cal ECM program benefits and community support services. This funding will allow ARMC, DBH, and DPH to continue to develop the ECM program, expanding enhanced healthcare access to populations and communities that have historically been under-served.
The County will continue to utilize the PATH CITED grant funding to support a whole-person care approach to address the medical and non-medical circumstances of high-need Medi-Cal beneficiaries enrolled in the ECM program. This includes hiring staff to train and on-board employees into the ECM program for all three departments, purchasing two vehicles for DPH to provide field services at locations best suited to support patients based on their needs, funding for continued consultation services contracted by ARMC, and indirect costs related to the administration of the PATH CITED grant. High-need Medi-Cal beneficiaries include individuals with complex medical needs, who are homeless and/or have serious mental illnesses, children with complicated medical conditions, and justice-involved populations.
On September 30, 2022, the County Chief Executive Officer (CEO) approved the submission of the grant application for PATH CITED, Round 1. As a result, on March 24, 2023, the County was awarded up to $2,781,853 in grant funding, with no matching funds required, for the performance period of January 1, 2022 through December 31, 2026.
On May 31, 2023, the County CEO approved the submission of the grant application for the PATH CITED, Round 2, but the County was not awarded any grant funding.
On February 15, 2024, the County CEO approved the submission of the grant application for PATH CITED, Round 3. On July 10, 2024, the County was awarded up to $352,638 in grant funding, with no matching funds required, for the performance period of August 20, 2024 through August 19, 2026. This award is being presented to the Board of Supervisors for acceptance at the earliest available meeting after completion of the required financial and legal reviews.
Under the grant terms, the County agrees to release and hold harmless Public Consulting Group LLC (the DHCS third-party contractor tasked with administration of the grant program) for any claim or liability arising out of or relating to the receipt of grant funds.
ARMC, DBH, and DPH recommend approval of this item to provide for the health, safety and social service needs of residents by supporting ECM program participants throughout the county.
