Recommendation
Approve Amendment No. 2 to Contract No. 25-102 (Award No. 6 H80CS00657-24-04), from the United States Department of Health and Human Services, Health Resource and Services Administration, for the Service Area Competition under the Health Center Program, accepting an amended grant award for continued operational support of the department’s Federally Qualified Health Centers, increasing the amount by $1,245,920, from $1,245,920 to $2,491,840, with no change to the period of March 1, 2025 through February 28, 2026.

FINANCIAL IMPACT
This item will not result in the use of Discretionary General Funding (Net County Cost). The amended grant award in the amount $1,245,920 from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2025-26 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
Every three years, health centers apply for HRSA funding under the Health Center Program’s Service Area Competition (Competition). Through the Competition, organizations compete for operational support funding which supports health care services to patient populations served by DPH’s Federally Qualified Health Centers (FQHC).
On August 8, 2023 (Item No. 35), and September 12, 2023 (Item No. 42), the Board of Supervisors (Board) approved Phase 1 and Phase 2, respectively, of the HRSA Competition grant application for FQHC operational support, in the amount $2,291,840 annually (amount included Phases 1 and 2), for the period of March 1, 2024 through February 28, 2027.
On March 12, 2024 (Item No. 30), the Board approved two grant awards, Contract Nos. 24-210 and 24-211 (Award No. 2H80CS00657-23-00 and No. 6 H80CS00657-23-01), for continued operational support of the FQHCs, in the amounts of $954,933 and $190,987, respectively, for the total amount of $1,145,920, for the period of March 1, 2024 through February 28, 2025. However, these grant awards were erroneously filed as separate contracts, rather than listing a single contract number and treating the second award as an amendment. In an effort to correct this issue, amendments and associated grant award amounts since the initial award have been included under Contract No. 24-211.
On May 7, 2024 (Item No.41), the Board approved Amendment No.1 to Contract No. 24-211 (Award No. 6 H80CS00657-23-02) in the amount of $100,000, for a combined total amount of $1,245,920, with no change to the period of March 1, 2024 through February 28, 2025.
Between August 14, 2024 and September 18, 2024, DPH received non-financial notices (Award Nos. 6 H80CS00657-23-05, 6 H80CS00657-23-06, and 6 H80CS00657-23-09) from HRSA informing DPH of administrative actions related to additional funding to be expected, deletion of the old address (606 E. Mill St. San Bernardino, CA 92408, and addition of the new address (590 N. D St, San Bernardino, CA 92401) of the FQHC located in San Bernardino.
On November 5, 2024 (Item No. 35), the Board approved Amendment Nos. 4, 5, and 6 to Contract No. 24-211 (Award Nos. 6 H80C00657-23-07 for $66,667, 6 H80CS00657-23-08 for. $28,500, and 6H80CS00657-23-10 for $37,000) from HRSA increasing the grant by $132,167, for a combined total of $2,559,267, with no change to the period of March 1, 2024 through February 28, 2025.
On January 21, 2025, DPH received non-financial notices (Award No. 6 H80CS00657-24-01) from HRSA informing DPH of administrative actions related to elimination of one or more grant conditions, with no change to the grant period of March 1, 2025 through February 28, 2026.
On February 11, 2025 (Item No. 31), the Board approved grant award, Contract No. 25-102 (Award No. 5 H80CS000657-24-00) from HRSA for the Service Area Competition under the Health Center Program for operational support of FQHC, in the amount of $1,038,267, for the period of March 1, 2025 through February 28, 2026.
On April 29, 2025 (Item No. 51), the Board approved Amendment No. 1 to Contract No. 25-102 (Award No. 6 H80CS00657-24-02) from HRSA increasing the grant by $207,653, for a combined total of $1,245,920, with no change to the period March 1, 2025 through February 28, 2026.
On June 24, 2025, DPH received a non-financial notice (Award No. 6 H80CS00657-24-03) from HRSA informing of administrative actions related to adding one or more grant conditions, with no change to the period of March 1, 2025 through February 28, 2026.
On July 12, 2025, DPH received notice of Amendment No. 2 to Contract No. 25-102, increasing the amount by $1,245,920, for a combined total of $2,491,840, for the period of March 1, 2025 through February 28, 2026. DPH will return to the Board to accept further funding from HRSA for the Service Area Competition, if and when it is received.
