RECOMMENDATION(S)
Recommendation
1. Accept and approve grant award (Grant Award No. 1 H2ECS45616-01-00), from the United States Department of Health and Human Services, Health Resources and Services Administration for the Health Center Program Service Expansion - School-Based Service Sites funding to support access to primary health care at school sites, in the amount of $200,000, for the period of May 1, 2022 through April 30, 2023.
2. Designate the Chairman of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to accept any subsequent non-substantive amendments in relation to the Health Center Program Service Expansion - School-Based Service Sites grant award (Grant Award No. 1 H2ECS45616-01-00) on behalf of the County, subject to review by County Counsel.
3. Direct the Chairman of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to transmit all non-substantive amendments in relation to the Health Center Program Service Expansion - School-Based Service Sites grant award (Grant Award No. 1 H2ECS45616-01-00) to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The grant award in the amount of $200,000 from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) for the Health Center Program Service Expansion - School-Based Service Sites (SBSS) funding does not require a local match. Approval of the necessary budget adjustments to the Department of Public Health’s (DPH) 2022-23 budget is not requested at this time, but will be included on a future quarterly countywide budget report presented to the Board of Supervisors (Board) for approval.
BACKGROUND INFORMATION
HRSA is providing Health Center Program SBSS funding to expand and support access to primary health care services at school sites. DPH will use the SBSS funding to increase access to health care services for vulnerable/underserved populations. DPH plans to collaborate with school based entities to initiate a new service model of providing primary care and dental services. The identification of the school based service sites will be based on the capacity of the collaborative partners to accommodate the service model in its shared space, as well as its proximity and logistical alignment with the Valley regions and High Desert region of DPH’s Federally Qualified Health Center (FQHC) locations. Close proximity to the FQHC’s will provide increased access to expanded health care services and allow referral opportunities to the FQHC’s for family members as needed. Once this new service model is established in the identified regions, and continued and sustained funding support is provided, DPH anticipates expanding this service in the future to other areas in proximity to FQHC locations in San Bernardino and Adelanto.
DPH will provide primary healthcare services at school-based services sites that will include well-child check-ups, screenings, immunizations, treatment for acute illness such as the flu, as well as chronic disease management for asthma and diabetes. DPH will also provide basic dental screenings, oral prophylaxis, fluoride and sealant treatments; recommendations for preventative intervention; and oral hygiene instruction and health education.
On March 22, 2021 and April 19, 2021, the County Administrative Office authorized the submission of Phase One and Phase Two applications, respectively, to HRSA for the Health Center Program Service Expansion - SBSS funding, in an amount of $200,000.
On May 3, 2022, DPH received the notice of award (Grant Award No. 1 H2ECS45616-01-00) from HRSA for the Health Center Program Service Expansion - SBSS funding, in the amount of $200,000, for the period of May 1, 2022 through April 30, 2023.
This item is being presented at this time, as this is the first available date following the required operational, fiscal, and legal reviews. Approval at this time will not impact DPH’s Health Center Program Expansion services or activities.
