Recommendation

Approve and authorize the submission of the 2026 Non-Competing Continuation Progress Report to the United States Department of Health and Human Services, Health Resources and Services Administration, to receive continued funding from the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant, which includes the increased amount of $224,363 from $9,460,867 to $9,685,230, for the period of March 1, 2026 through February 28, 2027.

FINANCIAL IMPACT

This item will not result in the use of Discretionary General Funding (Net County Cost). Submission of the 2026 Non-Competing Continuation Progress Report (Report) to the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) will allow the Department of Public Health (DPH) to continue to receive Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant (Ryan White Part A) funding, up to $9,685,230, for the period of March 1, 2026 through February 28, 2027. Ryan White Part A funding does not require a local match. Adequate appropriation and revenue have been included in DPH’s 2025-26 budget and will be included in future recommended budgets.

 

BACKGROUND INFORMATION

The Ryan White Program was enacted in 1990 to provide federal funding for comprehensive primary healthcare and support services for persons living with HIV/AIDS. Ryan White Part A provides funding to areas disproportionately impacted by the disease with a goal of improving health outcomes for persons living with HIV who are also low-income, uninsured, or underserved. Ryan White Part A services provided through subcontractors include medical care, medical case management, mental health services, dental care, substance abuse counseling services, early intervention services, medical nutrition therapy, transportation, housing, and food assistance.

 

DPH is the acting lead agency for San Bernardino and Riverside Counties. DPH administers the grant funds, including the development and submission of the annual grant application, acceptance of the grant award, procurement of the service contracts, and assurance of compliance with federal grant requirements.

 

Ryan White Part A is an annually funded program with a three-year performance period. The current performance period is March 1, 2025 through February 29, 2028. A competitive application is submitted for the first year of funding, followed by Reports for years two and three, allowing DPH to continue receiving additional funding for the Ryan White Part A program. The Report serves as an accounting of expenditure and provides an opportunity to capture changes to specific information provided in the competitive application, such as staffing plans, activities and services.    

 

On September 24, 2024 (Item No. 55), the Board of Supervisors (Board) approved and authorized the submission of a grant application for Ryan White Part A funding, in the amount of $9,460,867, for the period of March 1, 2025 through February 29, 2028.

 

On March 11, 2025 (Item No. 35), the Board approved and accepted Agreement No. 25-146 (Grant Award No. 2 H89HA00032-32-00) from HRSA for Ryan White Part A funding in the amount of $1,662,168, for the period of March 1, 2025, through February 28, 2026.

 

On August 5, 2025 (Item No. 60), the Board approved Amendment No. 1 to Agreement No. 25-146, increasing the amount by $2,444,965, from $1,662,168 to $4,107,133, for the period of March 1, 2025 through February 28, 2026.

 

Approval of this item would allow DPH to receive continued Ryan White Part A funding for the second year of the grant performance period. DPH will return to the Board to request acceptance of additional Ryan White Part A funding, if and when received. In 2025, HRSA extended the baseline cap of funding for 2026-27 Ryan White Part A funding that DPH could request. Funding was increased by $224,363 from $9,460,867 to $9,685,230. Therefore, the recommended Report reflects the higher baseline amount of $9,685,230.

 

This item is being presented at this time as this is the first date available following the required operational, fiscal, and legal review. Approval at this time will not impact DPH’s services or activities.
