Amendments to Contracts for Medical Care and Support Services under Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program

RECOMMENDATION(S)
Recommendation
Approve amendments to the following contracts to provide medical care and support services under the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program, increasing the total amount of the contracts by $16,069,052, from $21,945,041 to $38,014,093, revising the scope of work and budgets, and extending the contract for two years, for the total period of March 1, 2021 through February 28, 2026:
1. AIDS Healthcare Foundation, Amendment No. 3 to Contract No. 20-1178, increasing the contract by $1,261,610, from $1,808,465 to $3,070,075.
2. County of Riverside, Department of Public Health, Amendment No. 3 to Contract No. 20-1180, increasing the contract by $1,508,717, from $2,396,825 to $3,905,542.
3. Desert AIDS Project dba DAP Health, Amendment No. 3 to Contract No. 20-1181, increasing the contract by $6,428,315, from $8,108,473 to $14,536,788.
4. Foothill AIDS Project, Amendment No. 3 to Contract No. 20-1182, increasing the contract by $5,039,691, from $7,080,897 to $12,120,588.
5. SAC Health System, Amendment No. 4 to Contract No. 20-1183, increasing the contract by $1,035,231, from $1,501,821 to $2,537,052.
6. Young Scholars for Academic Empowerment dba TruEvolution, Inc., Amendment No. 3 to Contract No. 20-1184, increasing the contract by $795,488, from $1,048,560 to $1,844,048.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The amendments increase the total contract amounts by $16,069,052. These increases are funded by the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant (Ryan White Part A) Program and Minority AIDS Initiative (MAI), which will be fully reimbursed by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA). Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2023-24 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
DPH has received funding for the Ryan White Part A Program since 1994. Acting as the lead agency for the Transitional Grant Area of San Bernardino and Riverside counties, DPH oversees and administers the grant funds and assures compliance with grant requirements. Funding for the Ryan White Part A Program provides a comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with Human Immunodeficiency Virus (HIV) and their families. MAI funding is used specifically to assist African American and Hispanic individuals with HIV with linkage to and retention of care. The Inland Empire HIV Planning Council (IEHPC) is the planning body appointed by the Board of Supervisors (Board); it is mandated by Federal law to set service priorities for funding allocations for the Ryan White Part A Program. Services include medical and dental care, mental health services, substance abuse treatment, medical nutrition therapy, early intervention services, case management, transportation, psychosocial support, emergency financial assistance, housing, and food assistance.
Approval of this item increases the amount of funding to the contractors referenced in Recommendation and uses the extension clauses to add two years to the term.
On December 15, 2020 (Item No. 55), as a result of a formal procurement, the Board approved contracts with the agencies listed in the Recommendation and Borrego Community Health Foundation (Borrego), in the amount of $21,487,377, for the period of March 1, 2021 through February 29, 2024.
On July 27, 2021 (Item No. 36), the Board approved Amendment No. 1 to the contracts listed in the Recommendation and Borrego, decreasing the total contract amount by $750,948 due to a reduction in funding by HRSA allocated for this program, from $21,487,377 to $20,736,429, with no change to the contract period of March 1, 2021 through February 29, 2024.
On November 16, 2021 (Item No. 42), the Board approved Amendment No. 2 with Borrego and SAC Health System, adjusting the budget and scope of work, with no change to the total contract amount of $2,459,340, or the contract period of March 1, 2021 through February 29, 2024.
On March 24, 2022, the County terminated Contract No. 20-1179 at the request of Borrego. As a result of this termination, the value of the Borrego contract, in the amount of $1,359,629, has been removed from the summary data below.
On December 6, 2022 (Item No. 35), the Board approved Amendment No. 3 with SAC Health System and Amendment No. 2 with the rest of the agencies listed in the Recommendation, increasing the total amount of the contracts by $2,568,241 from $19,376,800 to $21,945,041, revising the scope of work and budgets, with no change to the contract period of March 1, 2021 through February 29, 2024.
Contractor performance will be measured by review of monthly reports, invoices, and supporting documentation to ensure payments are for approved services. A review of required data submission and an annual site visit by County staff will be conducted to monitor compliance with the administrative, program, and fiscal terms and conditions of the contract, with special attention to the Ryan White Part A payer of last resort requirement.
