Approve Amendment No. 2 to Revenue Contract No. 22-537 (State Revenue Agreement No. AD-2223-12) with the California Department of Aging to provide Aging and Disability Resource Connection Program services, updating Exhibit A: Scope of Work, Exhibit B: Budget Detail, Payment Provisions, and Closeout, and Exhibit D: Special Terms and Conditions, increasing the total contract amount by $631,113, from $1,464,039 to $2,095,152, and extending the term one year, for a total contract period of July 1, 2022 through June 30, 2025.

FINANCIAL IMPACT

This item does not require additional Discretionary General Funding (Net County Cost).  The increased funding of $631,113 for Amendment No. 2 (Amendment) to Contract No. 22-537 (Contract) from the California Department of Aging (CDA) for the Aging and Disability Resource Connection (ADRC) Program is 100% state funded.  Adequate appropriation and revenue are included in the Department of Aging and Adult Services’ (DAAS) 2024-25 recommended budget.

 

BACKGROUND INFORMATION

California Senate Bill 80 established the first state ADRC Infrastructure Grants Program in 2019.  The ADRC program develops coordinated networks of programs and services (No Wrong Door systems) to assist older adults, people with disabilities, and caregivers in navigating the complex system of Long-Term Services and Supports (LTSS), enabling them to pursue their personal goals and preferences for healthy aging.  The ADRC program assists in enabling consumers to access all LTSS programs through one agency, organization, coordinated network, or portal. 

 

ADRC core services and functions include enhancing information and referral services by searchable resource systems, warm transfers, and referrals between a wide array of organizations providing LTSS programs and services.  Additionally, ADRC provides options counseling to support person-centered planning on needed services and available programs. It also offers short-term coordination for the purpose of stabilizing a situation in which a person may be at risk and transitional services supporting the coordination of transitioning from health care facilities to home.

 

The recommended Amendment includes a new funding allocation of $631,113 for 2024-25, extends the term for one additional year, and extends the expenditure deadline for revenue previously allocated for 2023-24, in the amount of $672,624, from June 30, 2024 to June 30, 2025.  Additionally, this Amendment includes revision and replacement of Exhibits A, B, and D.  Exhibit A was revised to update an email address and correct outline numbering.  Exhibit B was revised to make several minor budget clarifications, and Exhibit D was revised to update a new equipment/property process, update CDA’s branch name, and clarify department contact information.

 

On June 28, 2022 (Item No. 3), the Board of Supervisors (Board) approved the Contract in the amount of $659,400, to provide ADRC program services, for the period of July 1, 2022 through June 30, 2023.  In addition, this item adopted Resolution No. 2022-129 authorizing the Chairman of the Board of Supervisors, the Chief Executive Officer, or the DAAS Director to execute all documents, including any subsequent non-substantive amendments, in relation to this Contract, on behalf of the County, subject to review by County Counsel.

 

On June 27, 2023 (Item No. 6), the Board approved Amendment No. 1 to the Contract, to provide ADRC program services, increasing the total amount by $804,639, from $659,400 to $1,464,039, and extending the Contract period one year, for a total period of July 1, 2022 through June 30, 2024.
