RECOMMENDATION(S)

Recommendation

1.                     Approve Amendment No. 1, effective July 1, 2021, to Revenue Contracts with the following agencies to provide public health nursing services for the Screening, Assessment, Referral and Treatment Program, increasing the total combined contract amount by $247,301, from $233,162 to $480,463, and extending the contracts an additional year, for the total contract period of July 1, 2020 through June 30, 2022:

a.                     San Bernardino County Superintendent of Schools California Association of Health & Education Linked Professions dba Desert/Mountain Children’s Center, Contract No. 20-399, increase by $86,816, from $83,335 to $170,151.

b.                     Victor Community Support Services, Inc., Contract No. 20-400, increase by $86,817, from $82,125 to $168,942.

c.                     West End Family Counseling Services, Inc., Contract No. 20-401, increase by $73,668, from $67,702 to $141,370.

2.                     Authorize the Director or Interim Director of Public Health to execute and sign the Public Health Nurse Case Management Services Contract and the termination of Network Access forms, as required by the San Bernardino County Superintendent of Schools Desert/Mountain Children’s Center contract for each employee requiring access to the School’s data network.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The Department of Public Health (DPH) will receive revenue in the amount of $247,301 in 2021-22 to provide nursing case management services. Adequate appropriation and revenue will be included in DPH’s 2021-22 recommended budget.

 

BACKGROUND INFORMATION

DPH is responsible for providing a variety of services throughout San Bernardino County aimed at promoting and encouraging healthy lifestyles. Since 2006, one of the services DPH provides is nursing case management services. Under the terms of the recommended contracts, Public Health nurses will provide nursing case management services for the agencies listed in the Recommendation, such as care coordination and consultation to ensure children/client and their families are partnered with appropriate agencies.

 

The Screening, Assessment, Referral, and Treatment (SART) program strives to ensure children (0 to 5 years of age) who are experiencing social, physical, cognitive, behavioral, developmental, and/or psychological issues are raised in a safe and nurturing home, and that children and families achieve optimal health and development. The SART program serves this at-risk population by utilizing a transdisciplinary approach, which includes screening, assessment, referral and treatment by clinicians, public health nurses, therapists, and pediatricians.

 

DPH participates with Children’s Network, Child and Families Commission for San Bernardino County (First 5), Behavioral Health, Children and Family Services, and community partners to support the SART program. DPH’s role is to provide comprehensive public health nursing case management services to ensure children have complete access to all medical, dental, and behavioral health services, including the provision of information to assist families in accessing resources available at the SART Clinical Center and/or Inland Regional Center.

 

For the period of July 1, 2020 through April 30, 2021, DPH has served 111 children at West End Family Counseling Services, Inc.  DPH nursing staff vacancies since July 2020 have limited DPH’s capacity to provide services for the other two agencies thus far in the current fiscal year.

 

San Bernardino County Superintendent of Schools (SBCSS) California Association of Health & Education Linked Professions Desert/Mountain Children’s Center requires two additional forms be completed as part of the contract. The contract contains provisions relating to the use of the SBCSS Desert/Mountain Children’s Center Data Network by County of San Bernardino Employees (i.e., DPH nurses). The Public Health Nurse Case Management Services Contract form and the Termination of Network Access forms must be completed for every employee requiring access to the Data Network and must be signed by the employee and the Director of Public Health, on behalf of the County, subject to review by County Counsel.

 

PROCUREMENT

On June 9, 2020 (Item No. 37), the Board of Supervisors approved revenue contracts with the agencies listed in the Recommendation to provide nursing services for the Screening Assessment, Referral and Treatment Program, in the total combined contract amount of $233,162 for the period of July 1, 2020 through June 30, 2021.
