Recommendation
Approve contracts with the following vendors on an as-needed, fee-for-service basis for post-acute care services in the aggregate not-to-exceed amount of $3,500,000 for the period of April 1, 2025 through March 31, 2030:
1. Mt. Rubidouxidence OPCO, LLC dba Jurupa Hills Post-Acute
2. Watermanidence OPCO, LLC dba Waterman Canyon Post-Acute

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost) as the total aggregate cost of $3,500,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2024-25 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
Approval of these contracts will allow ARMC to develop a panel of skilled nursing facilities (SNF) to which ARMC may transfer certain indigent patients who no longer meet the criteria for inpatient admission to an acute general care hospital and have the need for SNF services, including rehabilitative, wound, IV antibiotic therapy, dietary, sitter for behavioral patients, and other therapies who have been prescribed these services as a result of an illness, accident, surgery, or disability that do not have the financial resources to pay for their additional post care.
ARMC has been utilizing SNFs since 2011, which allows ARMC to ensure patients requiring lower levels of care receive proper treatment while making beds at the hospital available for other patients who have a need for inpatient care.
ARMC recommends approval of the contracts with Mt. Rubidouxidence OPCO, LLC dba Jurupa Hills Post-Acute (Jurupa Hills) and Watermanidence OPCO, LLC dba Waterman Canyon Post-Acute (Waterman Canyon) to provide for the safety, health and social service needs of County residents by allowing ARMC to reduce costs, improve quality of care, and increase overall reimbursement to ARMC.
PROCUREMENT
On October 30, 2024, Request for Proposal (RFP) No. ARMC125-ARMC-5632 to solicit proposals from interested and qualified vendors to provide post-acute care. The RFP was posted on the County’s Electronic Procurement Network (ePro).
Two proposals were received by the established deadline in response to the RFP. Proposals from the two vendors listed below met the minimum requirements and continued through the evaluation process.

The evaluation committee reviewed and evaluated the proposals based on the criteria released in the RFP. The criteria included: Qualifications and Experience, Technical Review, Reference, and Cost.
Based on the evaluation criteria, the evaluation committee determined that Jurupa Hills and Waterman Canyon both met the needs of ARMC and are being recommended for award to provide post-acute care services.
