Recommendation

Approve Amendment No. 1 to Contract No. 21-695 with Telecare Corporation for the provision of Forensic Assertive Community Treatment, updating standard contract language, and increasing the total contract amount by $556,107 from $6,311,250 to $6,867,357, with no change to the contract term of October 1, 2021, through September 30, 2026.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The contract increase of $556,107 for the Forensic Assertive Community Treatment (FACT) program will be funded by Mental Health Services Act and Medi-Cal Federal Financial Participation revenue. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2025-26 budget and will be included in the 2026-27 recommended budget. 

 

BACKGROUND INFORMATION

DBH is responsible for providing mental health and substance use disorder (SUD) services to county residents experiencing severe mental illness and/or SUD. An integral part of the service delivery system consists of assisting those who are historically high users of acute inpatient and crisis services.

 

The FACT program provides specialized, around-the-clock mental health services aimed at reducing recidivism in both jail and psychiatric hospitalization. This is accomplished through the delivery of mental health education and treatment; the development of independent living skills; and the provision of support services such as housing, social support, and outreach. These services foster a sense of community among participants, increasing overall public safety. The treatment program is community-based and provides supportive, intensive case management services to participants and family members at their residences,  including psychiatric consultations for those who are unable to attend office appointments. 

 

On September 21, 2021, (Item No. 10), the Board of Supervisors approved Contract No. 21-695 with Telecare Corporation for the FACT program in the amount of $6,311,250 for the period of October 1, 2021, through September 30, 2026.

 

Approval of this item will allow the FACT program to increase the number of individuals served in the final year of the contract term from 50 to 70 clients, for a total annual cost of $1,818,357 at an average cost of $25,976 per client. Due to a surge in Diversion referrals, this increase will ensure the program has sufficient capacity to accommodate clients without any service interruptions.

 

DBH will implement mechanisms to regularly review 1) client service data and progress, conduct site visits and annual monitoring to ensure performance and compliance standards of the contract(s) are met; 2) applicable claims data and claims for reimbursement to ensure fidelity and accuracy of service billing and optimization of Medi-Cal reimbursement in alignment with Contract terms and Department of Health Care Services billing requirements; and 3) provider invoices administratively and programmatically to ensure client and/or bed counts are accurate prior to payment processing.
