Body

COUNTY AND CHIEF EXECUTIVE OFFICER GOALS & OBJECTIVES

Operate in a Fiscally-Responsible and Business-Like Manner.

Provide for the Safety, Health and Social Service Needs of County Residents.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The updated annual cost of $351,819 will be fully funded by health center revenue consisting of Medi-Cal, Medicare, fee-for-service, and Public Health Services Act Section 330 grant funding. Adequate appropriation and revenue have been included in the 2023-24 Department of Public Health (DPH) budget.

BACKGROUND INFORMATION

DPH contracts with physicians to provide direct medical care to patients and serve as subject matter experts in their areas of expertise, such as primary care and reproductive health services. Physician support services include supervision and clinical oversight of mid-level practitioners (nurse practitioners and/or physician assistants), assisting in the development of policies and procedures, helping to establish productivity standards, and assisting DPH with the development of long-term strategic plans. Contracts with physicians are necessary as the work required by DPH programs may be part-time in nature and requires specific expertise in public health-related fields.

On May 22, 2018 (Item No. 93), the Board of Supervisors approved Contract No. 18-242 with Ryan Zane as a Public Health Physician - Specialty Services for an estimated annual cost of $271,628 ($192,608 Salary, $79,020 Benefits), for the period of June 9, 2018 through June 8, 2021.

On June 2, 2021, Amendment No. 1 to Employment Contract No. 18-242 was completed via delegated authority by the Director of Public Health, under the authority granted by the Board on May 22, 2018 (Item No. 93), to extend the contract term from June 9, 2018, to June 8, 2024, with no change to the total contract amount of $271,628.

DPH recommends amending Dr. Ryan Zane’s contract to revise the duties and responsibilities of contractor section, which will add new duties and requirements to the contract. These revisions would require Dr. Ryan Zane to attend DPH and Clinic Operations meetings and participate in pertinent training as required by DPH; engage in continual medical education opportunities to maintain core and specialty competencies; and provide coverage for the Medical Director as needed. The revision adds requirements that the contractor needs to possess and maintain active status for some of the following: Medical license in the State of California and in good standing with the Medical Board; credentialing required by the Federally Qualified Health Centers (FQHC); and Drug Enforcement Administration (DEA) controlled substance issued by the U.S. Department of Justice. DPH also recommends increasing Dr. Ryan Zane’s pay rate to Range 95C Step 15 to ensure pay equity.
