Recommendation

Approve Amendment No. 1 to Agreement No. 4400026262 with Abiomed Inc. for the purchase of Impella CP® Pumps and related accessories, increasing the contract amount by $600,000, from $150,000 to a total not-to-exceed amount of $750,000 and extending the contract term by two years, for a total term of July 10, 2024 through July 31, 2027.

FINANCIAL IMPACT

Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The cost of $600,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2024-25 budget and will be included in all future recommended budgets.

 

BACKGROUND INFORMATION

Amendment No. 1 to SAP Agreement No. 4400026262 (Agreement) with Abiomed Inc. (Abiomed) will allow ARMC to continue to purchase the Impella CP® Heart Pump Sets (Impella) and related accessories for ARMC’s Cardiovascular Program. The Impella is a heart pump that is used for patients with life threatening complete blockage of coronary arteries or cardiogenic shock. The Impella pumps blood out of the heart to the rest of the body which is necessary for Acute ST-Elevation Myocardial Infarction (STEMI) patients or patients having an acute myocardial infarction.

 

The Impella device plays a key role in helping ARMC achieve the STEMI Center and Primary Heart Attack Center designation because it supports critically ill patients during high-risk heart procedures. In STEMI (heart attack) cases, especially those where the heart is severely weakened, like in cardiogenic shock, the Impella provides essential blood flow support. It does this by assisting the heart in pumping blood, ensuring that oxygen-rich blood continues to circulate to vital organs.

 

When performing a primary percutaneous coronary intervention (PCI), which is a procedure to open blocked arteries, some patients may become unstable or have difficulty maintaining enough blood flow. In these situations, the Impella device stabilizes the patient and enhances the effectiveness of the procedure. Because of its ability to support patients through these high-risk interventions, having Impella technology available ensures that ARMC meets the necessary requirements to receive the STEMI Center and Primary Heart Attack Center designation, which recognizes hospitals equipped to provide the highest level of care for heart attack patients.

 

To enable ARMC to obtain the STEMI Center and Primary Heart Attack Center designation, the Impella is a necessary tool. Currently, ARMC has the monitoring equipment for Impella, and this Agreement allows ARMC to purchase additional Impella devices and related accessories, as needed, for patient care. 

 

On July 10, 2024, as the result of a non-competitive procurement, the Purchasing Agent executed an Agreement with Abiomed to purchase the Impella and related accessories in the not-to-exceed contract amount of $150,000 for the term of July 10, 2024 through July 9, 2025. 

 

ARMC recommends approval of this item, as the Impella and related accessories will provide for the safety and health needs of the County residents by providing tools to improve patient care for cardiovascular treatment.
