RECOMMENDATION(S)

Recommendation

Approve amendments to contracts with the following agencies for the provision of Promotores de Salud/Community Health Worker Program services, increasing the combined contract amount by $1,264,429, from $3,793,287 to $5,057,716, updating contract language and exercising the first option to extend the contracts by an additional year, for the total contract period of July 1, 2022, through June 30, 2026:

1.                     Amendment No. 1 to Contract No. 22-576 with Asian American Resource Center, increasing the amount by $125,000 from $375,000 to $500,000.

2.                     Amendment No. 1 to Contract No. 22-577 with Hearts & Lives, increasing the amount by $140,000 from $420,000 to $560,000.

3.                     Amendment No. 1 to Contract No. 22-578 with Riverside - San Bernardino County Indian Health, Inc., increasing the amount by $364,000 from $1,092,000 to $1,456,000.

4.                     Amendment No. 2 to Contract No. 22-579 with Pacific Clinics, increasing the amount by $135,000 from $405,000 to $540,000.

5.                     Amendment No. 1 to Contract No. 22-580 with Victor Community Support Services, increasing the amount by $500,429 from $1,501,287 to $2,001,716.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The recommended increase of $1,264,429 for the Promotores de Salud/Community Health Worker (PdS/CHW) program services will be funded by the Mental Health Services Act (MHSA) funding. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) recommended 2025-26 budget.

 

BACKGROUND INFORMATION

DBH is responsible for providing mental health and substance use disorder (SUD) services to San Bernardino County (County) residents experiencing severe mental illness and/or SUD. DBH provides a full range of mental health services for residents of the county through contracts with community-based organizations and County-operated clinics, with the goal of promoting prevention, intervention, treatment, education, recovery, and resiliency for individuals and families.

 

The PdS/CHW program focuses on the prevention portion of the Prevention and Early Intervention component of MHSA. Services are culturally and linguistically specific, and community-driven to reduce the stigma surrounding seeking behavioral health services for unserved, underserved, or inappropriately served populations. The program provides community education on mental health and SUD topics, promotes behavioral health prevention and wellness, and connects community members to local resources. Program success is measured through data from program participants. Agencies are required to administer quantitative and qualitative surveys to program participants with a minimum completion rate of 70% to ensure program goals are met. The target populations to be served include African American, Asian and Pacific Islander, Latino/LatinX, Lesbian, Gay, Bisexual, Transgender, Queer/Questioning, and Native American populations.

 

On June 28, 2022 (Item No. 32), as a result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 22-576 with Asian American Resource Center, in the amount of $375,000, Contract No. 22-577 Hearts & Lives, in the amount of $420,000, Contract No. 22-578 with Riverside - San Bernardino County Indian Health, Inc., in the amount of $1,092,000, Contract No. 22-579 with Uplift Family Services dba Pacific Clinics, in the amount of $405,000, and Contract No. 22-580 with Victor Community Support Services, in the amount of $1,501,287, for the period of July 1, 2022 through June 30, 2025.

 

On November 14, 2023 (Item No. 23), the Board approved Amendment No. 1 to Contract No. 22-579, reflecting the legal name change of Uplift Family Services dba Pacific Clinics to Pacific Clinics with no changes to the terms of the contract.

 

The PdS/CHW program is obligated to serve a minimum of 30,665 unduplicated participants at an estimated cost of $41 per participant. During 2023-24, the PdS/CHW program exceeded expectations by serving 74,533 unduplicated participants which reduced the cost per participant to $17.

 

DBH will implement mechanisms to regularly: 1) review client service data and progress, conduct site visits and annual monitoring to ensure performance and compliance standards of the contract(s) are met; 2) review applicable reimbursement claims and claims data to ensure fidelity and accuracy of service billing and optimization of Medi-Cal reimbursement in alignment with contract terms and Department of Health Care Services billing requirements; and 3) review provider invoices administratively and programmatically to ensure client and/or bed counts are accurate prior to payment processing.
