RECOMMENDATION(S)

Recommendation

1.                     Approve and accept grant award (Grant Award No. 5 H89HA00032-31-00) from the United States Department of Health and Human Services, Health Resources and Services Administration for the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant, in the amount of $2,622,603, for the period of March 1, 2024 to February 28, 2025.

2.                     Authorize the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to accept and execute any subsequent non-substantive amendments to the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant (Grant Award No. 5 H89HA00032-31-00), on behalf of the County, subject to review by County Counsel.

3.                     Direct the Director of the Department of Public Health to transmit all non-substantive amendments in relation to the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant (Grant Award No. 5 H89HA00032-31-00) to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The grant award (Grant Award No. 5 H89HA00032-31-00) in the amount of $2,622,603 from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) for the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant (Ryan White Part A Program) does not require a local match. Adequate appropriation and revenue are included in the Department of Public Health’s (DPH) 2023-24 budget and will be included in the 2024-25 recommended budget.

 

BACKGROUND INFORMATION

HRSA provides Ryan White Part A Program funds for assistance in developing and enhancing access to high quality, community-based medical care and support services for low-income individuals with HIV and their families. Services include medical care, medical case management, mental health services, dental care, substance abuse counseling services, early intervention services, medical nutrition therapy, transportation, housing, and food assistance.

 

The Ryan White Program was enacted in 1990 to provide federal funding for comprehensive primary healthcare and support services for persons living with HIV/AIDS. The Ryan White Part A Program grant provides funding for HIV/AIDS services to areas disproportionately impacted by the infection with individuals who are low-income, uninsured, and underserved to improve health outcomes. Minority HIV/AIDS Initiative services are designated to address disparities in access, treatment, care, and health outcomes for racial and ethnic minorities.

 

San Bernardino and Riverside counties first qualified for Ryan White Part A Program funding in 1994. DPH, acting as the lead agency for the two counties, administers the grant funds including development and submission of the annual grant application, acceptance of the grant award, procurement of service contracts, and assurance of compliance with federal grant requirements.

 

On September 21, 2021 (Item No. 29), the Board of Supervisors (Board) approved the application for Ryan White Part A Program funding, in the anticipated amount of $8,538,527, for the period of March 1, 2022 through February 28, 2023. HRSA has transitioned the Ryan White Part A Program from an annual competitive funded program to a multi-year funded program. The period of performance is three years, March 1, 2022 through February 28, 2025 for the current grant. A competitive application is required for the first year of funding, followed by Non-Competing Continuation progress reports for years two and three of the three-year grant performance period.

 

On March 1, 2022 (Item No. 42), the Board approved Contract No. 22-153 (Grant Award No. 2 H89HA00032-29-00) for the Ryan White Part A Program grant award in the amount of $2,719,797, for the period of March 1, 2022 through February 28, 2023, year one of the three-year grant performance period.

 

On July 12, 2022 (Item No. 25), the Board approved Amendment No. 1 to Contract No. 22-153 (Grant Award No. 2 H89HA00032-29-01) for the Ryan White Part A Program, increasing the grant award by $5,968,198, from $2,719,797 to $8,687,995, with no change to the period of March 1, 2022 through February 28, 2023.

 

On September 27, 2022 (Item No. 44), the Board approved the 2023 Non-Competing Continuation Progress Report in order to receive continued funding for the Ryan White Part A Program, in the amount of $9,122,393, for the period of March 1, 2023 through February 29, 2024, the second year of the three-year grant performance period.

 

On March 28, 2023 (Item No. 41), the Board approved Amendment No. 2 to Contract No. 22-153 (Grant Award No. 5 H89HA00032-30-00) for the Ryan White Part A Program grant award, in the amount of $1,520,647, for a new term of March 1, 2023 through February 28, 2024, the second year of the three-year grant performance period. The approved amendment reflects the transfer of unexpended funds in the amount of $89,455 from program year 2021-22 to program year 2022-23.

 

On May 23, 2023 (Item No. 57), the Board approved Amendment No. 3 (Grant Award No. 6 H89HA00032-30-01) and Amendment No. 4 (Grant Award No. 4 H89HA00032-30-02) to Contract No. 22-153 for the Ryan White Part A Program.  The purpose of Amendment No. 4 (Grant Award No. 4 H89HA00032-30-01), with no financial changes or grant funds awarded, was to update, correct and clarify the end date for the grant period as being February 29, 2024, as February 28, 2024, had been used prior to this issuance. The funding for Amendment No. 3 (Grant Award No. 6 H89HA00032-30-02) in the amount of $7,375,680, for the period of March 1, 2023 through February 29, 2024, allowed DPH to be reimbursed by HRSA for services rendered by existing Ryan White Part A Program contract providers retroactively to March 1, 2023. Although Amendment No. 2 was a separate grant award with a new term, DPH administratively processed the award as an amendment to the existing contract. As individual grant awards for a new term should be reflected as separate awards, the recommended award, and separate future awards, will be presented and processed as a new individual award.

 

On September 12, 2023 (Item No. 43), the Board approved the 2024 Non-Competing Continuation Progress Report in order to receive continued funding for the Ryan White Part A Program, in the amount of $9,341,142, for the period of March 1, 2024 through February 28, 2025, the third year of the three-year grant performance period.

 

On January 18, 2024, DPH received notice of Grant Award No. 5 H89HA00032-31-00 from HRSA for the Ryan White Part A Program in the amount of $2,622,603, for the period of March 1, 2024 through February 28, 2025. DPH will use these grant funds to continue to provide primary healthcare and support services to people living with HIV/AIDS. DPH will return to the Board to request acceptance of further funding for the Ryan White Part A Program, if and when received.
