RECOMMENDATION(S)

Recommendation

1.                     Authorize the Auditor-Controller/Treasurer/Tax Collector to post the necessary budget adjustments, as detailed in the Financial Impact section, for the purchase of CareManager, an electronic health record myAvatar module, from Netsmart Technologies, Inc., and additional staffing for the Enhanced Care Management Program and Providing Access and Transforming Health - Justice-Involved program (Four votes required).

2.                     Authorize the addition of two contract Psychiatrist II and two contract Clinical Therapist positions for the Department of Behavioral Health Enhanced Care Management Program.

FINANCIAL IMPACT

Approval of this item will not impact Discretionary General Funding (Net County Cost). The estimated cost of $2,310,063 for the recommended positions will be funded by the California Department of Health Care Services (DHCS) Providing Access and Transforming Health (PATH) - Justice-Involved Planning and Capacity Building Program (JI) - Round 3 grant (Grant) funds.

 

The Department of Behavioral Health (DBH) requests the Auditor-Controller/Treasurer/Tax Collector to post the following adjustments to their 2024-25 budget:

BACKGROUND INFORMATION

California State Assembly Bill 133 (Chapter 143; Statute 2021) authorized PATH JI funding to support the implementation of the full suite of statewide California Advancing and Innovating Medi-Cal (CalAIM) initiatives, such as JI, that builds on the successful work conducted under whole person care. The Grant builds capacity and expands access to necessary pre-release and post-release Medi-Cal services under CalAIM.

 

In June of 2023, DHCS released the application for Round 3 funding to support the implementation of CalAIM. The Grant funding is dedicated exclusively to justice-involved pre-release services, behavioral health linkage implementation, and Medi-Cal application and suspension processes.

 

On July 20, 2023, the Chief Executive Officer approved the submission of a Grant application. On August 14, 2023, DHCS notified DBH of approval of a provisional award in an amount not to exceed $2,310,063 and of the requirement to submit the Acknowledgement of Grant Terms and Conditions within 30 business days. DBH requested and was granted an extension by the third-party administrator to submit the required documents by November 30, 2023.

 

On October 3, 2023 (Item No. 12), the Board of Supervisors (Board) approved Agreement No. 23-1094 to accept the provisional Grant award, in the amount not to exceed $2,310,063, effective upon execution through March 31, 2026.

 

On January 14, 2025 (Item No. 27), the Board approved Contract No. 25-26 with NetSmart Technologies, Inc., to maintain the Behavioral Health Management Information System for DBH, in the amount of $22,479,308, for the period of July 1, 2024 through June 30, 2029.

 

The Grant funding will be used to purchase CareManager, from Netsmart Technologies, Inc., a module to the electronic health record (EHR) myAvatar. This module will serve as the EHR and case management system for the DBH’s Enhanced Care Management (ECM) and CalAIM JI programs.

 

The Grant will also fund up to two full-time equivalent (FTE) contract Psychiatrist II’s, two FTE contract Clinical Therapists, up to 15% FTE ECM Program Manager I, and purchase computer-related equipment for the CalAIM JI staff. The contract Psychiatrist II’s, from the CEP America/Vituity contract, will provide psychiatric evaluation and medication support services for adults and youth exiting a California Correctional Facility (CCF) and returning to the County to continue their healthcare services as they reintegrate into the community. The contract Clinical Therapists will conduct clinical assessments and professional-to-professional clinical handoffs for adults and youth exiting a CCF and returning to the County to continue their healthcare services as they reintegrate into the community.
