Recommendation
Approve non-financial Memorandum of Understanding with St. Mary Medical Center, for the continuity of care of Federally Qualified Health Center patients, for the period of November 1, 2020 through October 31, 2025.

BACKGROUND INFORMATION
The Department of Public Health (DPH) currently operates Federally Qualified Health Centers (FQHCs) located in Ontario, San Bernardino, Adelanto, and Hesperia. As such, they are required to follow the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) regulations. HRSA requires FQHC’s to enter into arrangements with hospitals to establish procedures for hospitalization, discharge planning, and patient tracking for FQHC patients receiving treatment at local hospitals.
The Memorandum of Understanding (MOU) with St. Mary Medical Center will establish the HRSA required procedures and exchange of information for any DPH FQHC patient receiving direct patient care or admitted to the hospital. DPH has been partnering with St. Mary’s Medical Center to serve FQHC clients in the Adelanto and Hesperia FQHCs for the last three years. Approval of this MOU will allow DPH to continue this collaborative for a maximum term of five years. The MOU is effective November 1, 2020 for a term of one year, and thereafter shall be renewed automatically for four successive one-year periods. Either party may terminate with or without cause upon 30 days written notice.
