Recommendation

Approve Amendment No. 3, effective November 5, 2019, to the following contracts for substance use disorder services through Community-Based Recovery Service Centers, updating standard contract language and increasing the total contract amount by $1,755,976, from $5,170,000 to $6,925,976, for the total contract period of July 1, 2015 through December 31, 2020:

1.                     Inland Valley Drug and Alcohol Recovery Services, Inc., Contract No. 15-319, increasing the total contract amount by $695,393, from $1,798,500 to $2,493,893.

2.                     Mental Health Systems, Inc., Contract No. 15-320, increasing the total contract amount by $697,500, from $2,222,000 to $2,919,500.

3.                     Rim Family Services, Inc., Contract No. 15-321, increasing the total contract amount by $35,640, from $694,320 to $729,960.

4.                     St. John of God Health Care Services, Contract No. 15-322, increasing the total contract amount by $327,443, from $455,180 to $782,623.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). The increase of $1,755,976 for Community-Based Recovery Service Centers will be funded by the Substance Abuse Prevention and Treatment Block Grant and Drug Medi-Cal. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2019-20 budget and will be included in the 2020-21 recommended budget.

 

BACKGROUND INFORMATION

DBH is responsible for providing mental health and substance use disorder services to San Bernardino County residents who are experiencing major mental illness and/or substance use disorders. DBH Substance Use Disorder Recovery Services (SUDRS) provide substance use disorder prevention and treatment services and education for communities and residents of San Bernardino County through contracts with community based organizations and County-operated clinics with the goal of promoting prevention, intervention, recovery, and resiliency for individuals and families. The recommended increase to the contracts listed in the Recommendation is necessary to allow DBH to implement a new service within the continuum of care’s Recovery Services. The new services are Medi-Cal billable and are required in all counties who elect to participate in the Drug Medi-Cal Organized Delivery System (DMC-ODS). These services will allow individuals who have completed a course of treatment and are experiencing triggers to relapse, have relapsed, or as a measure to prevent relapse, to obtain services to continue in and sustain their recovery. Upon approval of the amendments, it is anticipated that these agencies will provide approximately 24,467 recovery service hours at an average cost of $69.32 per service hour.

 

Community-Based Recovery Service Centers’ services include social activities, parenting education, aftercare and family groups, substance us disorder education, life skills, and smoking reduction/cessation. Recovery Services offered at the centers assist in encouraging and reinforcing positive behavioral changes through substance-free activities in a community forum.

 

The DMC-ODS is a component of the 1115 demonstration project implemented by the California Department of Health Care Services (DHCS). The DMC-ODS provides a continuum of care, enables more local control and accountability, provides greater administrative oversight, and creates utilization controls to improve care and efficient use of resources. It implements evidenced based practices in substance use disorder treatment, coordinates with other systems of care, and provides the patient with access to the care and system interaction needed in order to achieve sustainable recovery. The DMC-ODS demonstrates how organized substance use disorder care increases the success of DMC patients while decreasing other system health care costs.

 

PROCUREMENT

On June 16, 2015 (Item No. 24), as a result of procurement, the Board of Supervisors (Board) approved contracts with Inland Valley Drug and Alcohol Recovery Services, Inc., Mental Health Systems, Inc., Rim Family Services, Inc., and St. John of God Health Care Services, in the amount of $2,820,000, for the period of July 1, 2015 through June 30, 2018, with the option to extend for two one-year periods contingent upon funding and contract performance.

 

On June 12, 2018 (Item No. 18), the Board approved Amendment No. 1 to the contracts, increasing the total contract amount by $940,000 to $3,760,000, and exercising the first one-year option to extend contracts through June 30, 2019.

 

On May 21, 2019 (Item No. 28), the Board approved Amendment No. 2 to the contracts updating standard language, increasing the total contract amount by $1,410,000 to $5,170,000, and exercising the final option to extend the contracts through December 31, 2020.

 

Amendment No. 3 to the contracts for Community-Based Recovery Service Centers will increase the total contract amount by $1,755,976 to $6,925,976 and update standard contract language.  DBH will continue to monitor contract performance on a regular basis to ensure compliance standards are met.
