RECOMMENDATION(S)
Recommendation
1. Approve and accept Amendment No. 4 to Contract No. 24-211(Award No. 6 H80CS00657-23-07), from the United States Department of Health and Human Services, Health Resources and Services Administration for the Service Area Competition for continued operational support of the Federally Qualified Health Centers, increasing the grant award amount by $66,667, from $2,427,100 to $2,493,767, with no change to the period of March 1, 2024 through February 28, 2025.
2. Approve and accept Amendment No. 5 to Contract No. 24-211 (Award No. 6 H80CS00657-23-08), from the United States Department of Health and Human Services, Health Resources and Services Administration for the Service Area Competition for continued operational support of the Federally Qualified Health Centers, increasing the grant award amount by $28,500, from $2,493,767 to $2,522,267, with no change to the period of March 1, 2024 through February 28, 2025.
3. Approve and accept Amendment No. 6 to Contract No. 24-211 (Award No. 6 H80CS00657-23-10), from the United States Department of Health and Human Services, Heath Resources and Services Administration for the Service Area Competition for continued operational support of the Federally Qualified Health Centers, increasing the grant award amount by $37,000, from $2,522,267 to $2,559,267, with no change to the period of March 1, 2024 through February 28, 2025.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The amendments in the Recommendation to Contract No. 24-211, for the total amount of $132,167, from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA), do not require a local match. Currently, funding from this HRSA grant has been issued through partial awards due to pending congressional action on the Health Center Program appropriation. Adequate appropriation and revenue have been included in the Department of Public Health’s DPH’s 2024-2025 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
Every three years, health centers apply for funding under the HRSA Service Area Competition (SAC). Through SAC, organizations compete for operational funding, which supports health care services to patient populations served by Federally Qualified Health Centers (FQHC). DPH applied for and received a three-year grant award for a term of March 1, 2024 through February 28, 2027. This award will continue to support the department’s FQHCs in Adelanto, Hesperia, San Bernardino, and Ontario. Services include primary care, maternal health, pediatric care, HIV, treatment and prevention, reproductive health, dental and nutrition services, and diagnostic radiology to an estimated 13,075 adults and children annually.
On August 8, 2023 (Item No. 35), and September 12, 2023 (Item No. 42), the Board of Supervisors (Board) approved Phase 1 and Phase 2, respectively, of the HRSA grant Application for the Competition under the Health Center Program for FQHC operational support, in the amount of $2,291,840 annually (amount includes both Phases 1 and 2), for the period of March 1, 2024 through February 28, 2027.
On March 12, 2024 (Item No. 30), the Board approved two grant awards, Contract Nos. 24-210 and 24-211 (Award No. 2 H80CS00657-23-00 and No. 6 H80CS00657-23-01), in the amounts of $954,933 and $190,987, respectively, for the total amount of $1,145,920, for the period of March 1, 2024 through February 28, 2025. However, these grant awards were erroneously filed as separate contracts, rather than listing a single contract number and treating the second award as an amendment. In an effort to correct this issue, amendments and associated grant award amounts since the initial award have been included under Contract No. 24-211.
On May 7, 2024 (Item No. 41), the Board approved Amendment No. 1 to Contract No. 24-211 (Award No. 6 H80CS00657-23-02) in the amount of $100,000, for a combined total amount of $1,245,920, with no change to the period of March 1, 2024 through February 28, 2025.
On June 11, 2024 (Item No. 65), the Board approved Amendment No. 2 to Contract No. 24-211 (Award No. 6 H80CS00657-23-03) in the amount of $1,145,920, for a combined total of $2,391,840, with no change to the period of March 1, 2024 through February 28, 2025.
On July 23, 2024 (Item No. 46), the Board approved Amendment No. 3 to Contract 24-211 (Award No. 6 H80CS00657-23-04) in the amount of $35,260, for a combined total amount of $2,427,100, with no change to the period of March 1, 2024 through February 28, 2025.
Between August 14, 2024 and September 18, 2024, DPH received non-financial notices (Award Nos. 6 H80CS00657-23-05, 6 H80CS00657-23-06, and 6 H80CS00657-23-09) from HRSA informing of administrative actions related to additional funding to be expected, deletion of the old address (606 E. Mill St. San Bernardino, CA 92408, and addition of the new address (590 N. D St, San Bernardino, CA 92401) of the FQHC located in San Bernardino.
On September 11, 2024, DPH received Amendment No. 4 (Award No. 6 H80CS00657-23-07) from HRSA for additional funding in the amount of $66,667, for a combined total of $2,493,767, with no change to the budget period of March 1, 2024 through February 28, 2025.
On September 18, 2024, DPH received Amendment No. 5 (Award No. 6 H80CS00657-23-08) from HRSA for additional funding in the amount of $28,500, for a combined total of $2,522,267, with no change to the budget period of March 1, 2024 through February 28, 2025.
On September 19, 2024, DPH received Amendment No. 6 (Award No. 6 H80CS00657-23-10) from HRSA for additional funding in the amount of $37,000, for a combined total of $2,559,267, with no change to the budget period of March 1, 2024 through February 28, 2025.
This item is being presented at this time, as this is the first available date following the required operational, fiscal, and legal reviews. Approval of this item will not impact DPH’S FQHC services or activities.
