Approve Amendment No. 1 to Contract No. 24-156 with CarePort Health, LLC, an affiliate of WellSky Corporation, to upgrade the licenses for the CarePort learning management cloud services and add implementation services, increasing the contract amount by $573,981, from $500,000 to a total not to exceed amount of $1,073,981, with no change to the contract term of July 1, 2024 through June 30, 2029.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost) as the cost of $573,981 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2025-26 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
This Amendment reflects a change to the scope of work due to delayed implementation of the upgraded services, while also incorporating a one-time implementation fee of $38,250 associated with the CarePort Transition Option. This Amendment will allow ARMC to upgrade its current licenses to the CarePort Transition Option, which will provide additional features to ARMC’s current patient placement referral system, such as enhanced patient choices, post-acute authorization capabilities, and return data feed with the Epic Electronic Health System (current electronic medical record). The upgraded features will help improve patient satisfaction and reduce unnecessary length of stay by allowing ARMC staff to follow up with patients post discharge, whether in a skilled nursing facility or with home health. This, in turn, provides higher quality patient care for county residents.
On February 27, 2024 (Item No.16), as the result of solicitation No. ARMC123-ARMC-4950 , the Board of Supervisors (Board) approved Agreement No. 24-156 with CarePort Health, LLC (CarePort), an affiliate of WellSky Corporation, subject to the terms of Agreement No. 24-155 for the patient placement referral system in the amount of $100,000 annually for the term of July 1, 2024 through June 30, 2029. Due to implementation timing and system integration considerations, the upgraded CarePort Transition Option could not be implemented as originally anticipated, necessitating a revised operational start date and corresponding adjustment to the service term, due to Transforming Episode Accountability Model (TEAM) not going live until January 1, 2026.
ARMC recommends approval of the Amendment with CarePort, to allow ARMC to upgrade to the CarePort Transition Option, providing enhanced features to the existing patient placement referral system.
