RECOMMENDATION(S)
Recommendation
Approve an increase in funding allocation to the following Master Service Agreements with Stericycle, Inc., in the aggregate amount of $600,000, for the provisions of waste disposal services, increasing the total contract amount from a not-to-exceed aggregate amount of $2,900,000 to a not-to-exceed aggregate amount of $3,500,000, with no change to the term of the contracts as specified:
1. Agreement No. 21-412 relating to pharmaceutical waste disposal, with a term of June 8, 2021 through June 7, 2026.
2. Agreement No. 21-413 relating to regulated waste disposal, with a term of June 8, 2021 through June 7, 2026.
3. Agreement No. 21-414 relating to hazardous waste disposal, with a term of June 8, 2021 through June 7, 2026.
4. Agreement No. 21-741 relating to controlled substance disposal, with a term of October 5, 2021 through June 7, 2026.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The aggregate cost of $3,500,000 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2025-26 budget.
BACKGROUND INFORMATION
The increase in funding allocation will provide ARMC the ability to continue to utilize Stericycle, Inc. (Stericycle) to dispose of regulated medical, pharmaceutical, hazardous, and controlled substance waste in a safe and efficient manner. As a Level I Trauma Center, there has been a significant increase in the need for waste disposal services at ARMC which requires the addition of funding to pay for the services.
On June 8, 2021 (Item No. 15), the Board of Supervisors (Board) approved competitively procured Master Service Agreements Nos. 21-412, 21-413, and 21-414 for pharmaceutical waste, regulated waste, and hazardous waste disposal services, respectively, with Stericycle in a total aggregate amount not-to-exceed $2,500,000, for the term of June 8, 2021 through June 7, 2026. However, the agreement terms did not include a dollar limit on the services; instead, the agreements listed the costs based on units. The aggregate not-to-exceed amount of $2,500,000 was an internal County limit specified on the item and coversheet.
On October 5, 2021 (Item No. 14), the Board approved Master Service Agreement No. 21-741 with Stericycle for controlled substance waste disposal services, with a term of October 5, 2021 through June 7, 2026, and increased the total aggregate not-to-exceed amount for the master service agreements with Stericycle by $250,000, from $2,500,000 to $2,750,000.
On April, 29, 2025 (Item. No. 11) the Board approved an increase in funding allocation to the following Master Service Agreements with Stericycle, increasing the total aggregate not-to-exceed amount to $2,900,000, with no change to the term of the contracts:
1. Agreement No. 21-412 relating to pharmaceutical waste disposal
2. Agreement No. 21-413 relating to regulated waste disposal
3. Agreement No. 21-414 relating to hazardous waste disposal
The Board item inadvertently omitted Agreement No. 21-741.
ARMC recommends approval of the increase in funding allocation for the increase of pharmaceutical waste, regulated waste, controlled substance and hazardous waste disposal, as it will enable the hospital to provide for the safety, health and social services needs of County residents.
