RECOMMENDATION(S)
Recommendation
Approve amendments to the following contracts to provide medical care and support services under the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant Program, increasing the total amount of the contracts by $144,288, from $38,014,093 to $38,158,381, revising the scope of work and budgets, with no change to the contract period to March 1, 2021 through February 28, 2026:
1. Amendment No. 4 to Contract No. 20-1178 with AIDS Healthcare Foundation, increasing the contract by $139,888, from $3,070,075 to $3,209,963.
2. Amendment No. 4 to Contract No. 20-1180 with County of Riverside, Department of Public Health, increasing the contract by $210,520 from $3,905,542 to $4,116,062.
3. Amendment No. 4 to Contract No. 20-1181 with DAP Health, decreasing the contract by $804,238, from $14,536,788 to $13,732,550.
4. Amendment No. 4 to Contract No. 20-1182 with Foothill AIDS Project, increasing the contract by $105,210, from $12,120,588 to $12,225,798.
5. Amendment No. 5 to Contract No. 20-1183 with SAC Health System, increasing the contract by $600,970 from $2,537,052 to $3,138,022.
6. Amendment No. 4 to Contract No. 20-1184 with Young Scholars for Academic Empowerment dba TruEvolution, Inc., decreasing the contract by $108,062, from $1,844,048 to $1,735,986.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The amendments include an increase of $1,056,588 and a decrease for two contracts of $912,300, resulting in an offset overall increase of $144,288. The contracts are funded by the Ryan White HIV/AIDS Part A HIV Emergency Relief Grant (Ryan White Part A) Program and Minority AIDS Initiative (MAI), which will be fully reimbursed by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA). Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2024-25 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
DPH has received funding for the Ryan White Part A Program since 1994. Acting as the lead agency for the Transitional Grant Area of San Bernardino and Riverside counties, DPH oversees and administers the grant funds and assures compliance with grant requirements. Funding for the Ryan White Part A Program provides comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with HIV and their families. MAI funding is used specifically to assist African American and Hispanic individuals with HIV with linkage and retention to care and treatment. The Inland Empire HIV Planning Council (IEHPC) is the planning body appointed by the Board of Supervisors (Board) and mandated by Federal law to set service priorities for funding allocations for the Ryan White Program. Services include medical care, mental health services, dental care, substance abuse treatment, medical nutrition therapy, early intervention services, case management, transportation, psychosocial support, emergency financial assistance, housing, and food assistance.
On December 15, 2020 (Item No. 55), as a result of a formal procurement, the Board approved contracts with the agencies listed in the Recommendation and Borrego Community Health Foundation, in the amount of $21,487,377, for the period of March 1, 2021 through February 29, 2024. Borrego Community Health Foundation’s contract, in the amount of $1,359,629, was mutually terminated on March 24, 2022.
On July 27, 2021 (Item No. 36), the Board approved Amendment No. 1 to the contracts listed in the Recommendation and Borrego Community Health Foundation, decreasing the total contract amount by $750,948, from $21,487,377 to $20,736,429, for the contract period of March 1, 2021 through February 29, 2024.
On November 16, 2021 (Item No. 42), the Board approved Amendment No. 2 with Borrego Community Health Foundation and SAC Health System, adjusting the budget and scope of work, with no change to the total contract amount of $2,459,340, or the contract period of March 1, 2021 through February 29, 2024.
On December 6, 2022 (Item No. 35), the Board approved Amendment No. 3 with SAC Health System and Amendment No. 2 with the other agencies listed in the Recommendation, increasing the total amount of the contracts by $2,568,241 from $19,376,800 to $21,945,041, revising the scope of work and budgets, with no change to the contract period of March 1, 2021 through February 29, 2024.
On September 26, 2023 (Item No. 53) the Board approved Amendment No. 4 with SAC Health System and Amendment No. 3 with the rest of the agencies listed in the Recommendation, increasing the total amount of the contracts by $16,069,052 from $21,945,041 to $38,014,093, revising the scope of work and budgets, and extending the contract for two years, for the total period of March 1, 2021 through February 28, 2026.
Approval of the Recommendations will increase the aggregate amount of the contracts by $144,288, due to the final allocation received from HSRA. DPH reviewed the populations being served and determined DAP Health and Young Scholars for Academic Empowerment dba TruEvolution Inc. allocation required a decrease due to the current volume of patients they are serving. DAP Health funding has been reallocated to SAC Health System, while Young Scholars for Academic Empowerment dba TruEvolution Inc. funding has been reallocated to the remaining three contractors due to discontinuing Housing Services. Services to patients in communities of color, which include African Americans, Latinos, and American Indians under the initiative are not impacted, as funding is utilized from other Ryan White sources to ensure HIV-related health disparities continue to be addressed. Reducing disparities in care and improving health outcomes for persons living with HIV are the IEHPC service priorities.
