Recommendation
Approve Amendment No. 2 to Agreement No. 21-899 with iSchemaView, Inc., for Pulsara software integration, increasing the contract amount by $2,500, from $387,000 to $389,500, with no change to the contract term of January 8, 2019 through January 7, 2027, automatically renewing for up to five successive annual terms unless terminated by the County.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The additional cost of $2,500 is funded by State Medi-Cal, Federal Medicare, private insurances, and other departmental revenue. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2021-22 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
Approval of this amendment will allow ARMC to integrate Pulsara with iSchemaView’s RapidAI software applications. The iSchemaView imaging software allows ARMC to diagnose stroke patients faster with post-stroke processing software to help inform physicians if further surgery intervention is needed for large vessel occlusions (blockages or closures). This software is industry standard with area hospitals and required for comprehensive stroke designation for ARMC. Pulsara is a software that notifies physicians of stroke cases and enables access to critical patient post-stroke images on IOS (Apple) and Android devices. Time is of the essence when treating stroke patients. By integrating iSchemaView’s RapidAI software with the Pulsara software, physicians and staff working with stroke patients can obtain all relevant images to determine the severity of stroke and best treatment via one software access point. The software application sends group messages to all staff working with the patient, resulting in faster response time for treatment through a secured system that meets Health Insurance Portability and Accountability Act (HIPAA) requirements.
On November 30, 2018, the Purchasing Agent executed the original Agreement No. 4400011060 with iSchemaView for the purchase of imaging software to assist with the treatment of stroke patients in the amount of $149,500. On December 7, 2021 (Item No. 8), the Board of Supervisors approved Amendment No. 1 to Agreement No. 21-899 (previously referenced as Agreement No. 4400011060) with iSchemaView for imaging software to assist with rapid treatment of stroke patients, increasing the contract amount by $237,500, from $149,500 to $387,000, and increasing the contract term by five years for a total contract period from January 8, 2019 through January 7, 2027, automatically renewing for up to five successive annual terms unless terminated by the County.
ARMC recommends approval of this request to allow ARMC to maintain comprehensive stroke certification and continue efforts to quickly service stroke patients in San Bernardino County which can shorten response times for possible surgery.
PROCUREMENT
Purchasing approves this continued non-competitive procurement based on functional specifications as the Pulsara software integration with iSchemaView is needed for ARMC to maintain its comprehensive stroke certification, and to shorten critical treatment times for stroke patients through easy transfer between area hospitals.
