Approve Amendment No. 2 to Contract No. 21-218 with Stacy Wood, Ph.D., A Psychological Corporation, to provide geriatric assessment services to older adults and individuals with disabilities, extending the contract term by one year, for a new total term of April 1, 2021, through March 31, 2026, with no change to the contract amount of $250,000.

FINANCIAL IMPACT
This item does not impact Discretionary General Funding (Net County Cost). The contract amount of $250,000 for geriatric assessment services is 100% Federally funded through American Rescue Plan Act revenue through September 30, 2025 which the state has already received. Funding from October 1, 2025 through March 31, 2026 will be 50% federal funds and 50% 2011 Realignment funding through the Adult Protective Services (APS) Allocation, and subject to availability of funds. Should these funds be delayed, services under this contract will be suspended after September 30, 2025 until further notice. Adequate appropriation and revenue have been included in the Human Services Administrative Claim 2024-25 budget and will be included in the 2025-26 recommended budget
BACKGROUND INFORMATION
Every year, thousands of older adults (aged 60 and older) and dependent adults (aged 18 through 59) are victims of abuse and/or neglect. The County APS is mandated to conduct investigations of older and dependent adults who are unable to meet their own needs, or are victims of abuse, neglect, or exploitation. APS staff evaluates cases and provides services including, but not limited to, advocacy, crisis intervention, case management, referrals and linkage to appropriate services. When APS staff identify potential concerns about the clients’ ability to make decisions and perform daily, a professional capacity assessment is necessary to determine next steps in the investigation.
Approval of Amendment No. 2 to Contract No. 21-218 (Contract) with Stacy Wood, Ph.D., A Psychological Corporation (Stacy Wood), continues the provision of geriatric and/or capacity assessment, including travel throughout the county to meet clients where they are located. The assessments include, but are not limited to, mental status examinations, decision-making capacity evaluations, geriatric depression assessments, and activities of daily living assessments. These assessments help APS to determine and justify actions that may become necessary to protect these vulnerable individuals, such as a conservatorship referral. This one-year extension does not require an increase as there is over $90,000 remaining of the $250,000 contract total. The cost of assessments varies based on level of evaluation required. An average of 34 assessments have been completed annually by Stacy Wood, at an average cost of $963 per assessment.
On March 23, 2021 (Item No. 5), the Board of Supervisors (Board) approved the Contract with Stacy Wood, in the amount of $250,000 for the three-year term of April 1, 2021, through March 31, 2024, with the option of two one-year extensions.
On March 26, 2024 (Item No. 9), the Board approved Amendment No. 1 to the Contract with Stacy Wood to provide geriatric assessment services to older adults and individuals with disabilities, extending the Contract term by one year for a total contract period of April 1, 2021, through March 31, 2025, with no change to the Contract amount of $250,000.
Stacy Wood’s performance will continue to be measured by reviewing monthly reports and annual site visits by County staff to monitor compliance with the administrative, fiscal, and program requirements of the Contract. The Contract may be terminated by the County immediately if funds are not available, and without cause by serving a written notice to Stacy Wood 30 days in advance of termination.
