Recommendation
Approve non-financial Contract with WelbeHealth Inland Empire Program of All Inclusive Care for Elderly, LLC, for the exchange of information regarding mutual customers to promote retention and successful renewals for Medi-Cal coverage, for the period of August 5, 2025 through August 4, 2030.

BACKGROUND INFORMATION
Under the terms of the recommended contract, WelbeHealth (Welbe) Inland Empire Program of All Inclusive Care for Elderly (PACE), LLC will provide the Transitional Assistance Department (TAD) with a complete list of San Bernardino County (County) Welbe Members (Members) twice per month. TAD will update contact and demographic information received from Welbe to ensure Medi-Cal beneficiaries receive accurate and timely information regarding their Medi-Cal eligibility.
TAD administers public assistance programs in the County, including Medi-Cal, California’s Medicaid program serving low income and disabled individuals. Eligibility for Medi-Cal must be reevaluated on an annual basis. Failure to complete and return an annual renewal to TAD can result in loss of benefits for individuals and families.
Welbe is a prepaid Medi-Cal managed care health plan contracted with the California Department of Health Care Services. Welbe’s PACE provides full-service care to help seniors age well and stay in their homes and communities by coordinating and providing comprehensive services, including, but not limited to, medical care and community-based services such as rehabilitation therapies, engagement programs, nutritional support, transportation, in home services, and social services.
Welbe will conduct general outreach to Members to encourage the completion and return of their renewal packets or provide needed information to TAD, to the extent permitted by federal and state law and this contract.
