RECOMMENDATION(S)

Recommendation

1.                     Approve and accept Amendment No. 1 to Contract No. 23-308 (Award No. 4 H2ECS45616-02-04) from the United States Department of Health and Human Services, Health Resources and Services Administration for the Health Center Program Service Expansion - School Based Service Sites to support access to primary health, extending the grant term an additional year, for a total term of May 1, 2023 through April 30, 2025, with no change to the contract total amount of $200,000.

2.                     Authorize the Chair of the Board of Supervisors, Chief Executive Officer, or Director of the Department of Public Health to execute any subsequent non-substantive amendments in relation to Amendment No. 1 to Contract No. 23-308 on behalf of the County, subject to review by County Counsel.

3.                     Direct the Director of the Department of Public Health to transmit all non-substantive amendments in relation to Amendment No. 1 to Contract No. 23-308 to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT

This item does not impact Discretionary General Funding (Net County Cost). Extending the performance of the grant period by one year, with no change to the amount of $200,000 does not require a local match. Adequate appropriation and revenue are included in the Department of Public Health’s (DPH) 2024-25 budget.

 

BACKGROUND INFORMATION

The United States Department of Health and Human Services, Health Resources and Services Administration for the Health Center Program Service Expansion (HRSA) - School Based Service Sites (School Based Sites) provides funding to expand access to health center services by increasing the number of patients who access comprehensive primary health care through school-based locations.

 

DPH, in partnership with the Preschool Services Department, provides primary healthcare services at two school-based sites in Ontario and Apple Valley.  Services include well-child check-ups, screenings, immunizations, treatment for acute illness such as influenza, as well as chronic disease management for asthma and diabetes. DPH also provides basic dental screenings; oral prophylaxis, fluoride and sealant treatments; recommendations for preventative intervention; and oral hygiene instruction and health education.

 

On April 25, 2023 (Item No. 47) the Board approved Contract No. 23-308 Grant Award (Grant Award No. 5 H2ECS45616-02-00), with HRSA for School Based Sites to support access to primary health care, in the amount of $200,000, for the period of May 1, 2023 through April 30, 2024.

 

On October 9, 2024, DPH received Amendment No. 1 (Award No. 4 H2ECS45616-02-04) indicating HRSA would be extending the performance grant period for one year, for a total term of May 1, 2023, through April 30, 2025, with no change in the total contract amount. Though DPH was able to continue school-based primary care services after April 30, 2024, under other funding from the HRSA Service Area Competition grant, this term extension will allow DPH to utilize remaining unspent funding and be retroactively reimbursed. 

 

This item is being presented at this time as this is the first available date following the required operational, fiscal, and legal reviews. Approval of this item will not impact DPH’s school-based primary care services or activities.
