This item does not impact Discretionary General Funding (Net County Cost). The revenue contract with the Children and Families Commission for San Bernardino County (First 5) will reimburse the Department of Behavioral Health (DBH) in the amount of $7,823,368 for the 0-5 Comprehensive Treatment Services (CTS) program and related staffing (three essential staff), and administrative costs. Estimated cost of services and staffing based on anticipated number of clients served are detailed in the table below:

Adequate appropriation and revenue have been included in DBH’s 2023-24 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
DBH is responsible for providing behavioral health and/or substance use disorder services to San Bernardino County (County) residents who are experiencing severe and persistent mental illness, substance use disorders, and other addictions. An integral part of the service delivery system consists of County-operated and contracted service providers delivering a variety of mental health treatment services, including CTS, which is a Mental Health Services Act-Prevention and Early Intervention program administered through the DBH Children and Youth Connection Services unit. DBH has been collaborating with First 5 to provide these services since 2004.
First 5 works collaboratively with the community and agencies to promote, support, and improve the early development of children from the prenatal stage through age five. DBH receives revenue from First 5 for the reimbursement of program services provided by DBH through 0-5 CTS contract providers. Under the terms of the recommended revenue contract, DBH will administer and manage the 0-5 CTS program for the delivery of services and First 5 will reimburse DBH for program services and related administrative costs. The 0-5 CTS program consists of Screening, Assessment, Referral and Treatment (SART) and Early Identification and Intervention Services (EIIS).
The SART program provides services to children ages 0-5 who have experienced physical, sexual, or emotional abuse; experienced premature birth, poor maternal nutrition, or prenatal exposure to alcohol or other drugs; family violence; family substance abuse; maternal mental illness; or been involved in the foster care system. This population has been diagnosed with mental health disorders that require specialty mental health services.
The EIIS program provides services to children/youth ages 0-9 who struggle with social-emotional disturbances or display developmental concerns that require less intensive, short-term interventions. Services focus on children identified as unserved, underserved, or who have been referred from SART or the universal screening process. Although EIIS provides services for children/youth ages 0-9, reimbursement from First 5 is provided for children/youth ages 0-5.
Since July 1, 2018, approximately 5,823 children have been served through SART and 4,025 have been serviced through EIIS. For 2023-24 through 2027-28, the expected number of children to be served per year is approximately 3,595. The cost per client to First 5 is $835 in 2023-24, $556 in 2024-25, $278 in 2025-26, $0 in 2026-27 and $0 in 2027-28. For this contract period, First 5 will begin systematically decreasing funding, and other DBH funding sources will be used to cover the difference, resulting in no funding for services from First 5 for the last two years of the contract.
Approval of the recommended revenue contract with First 5 will allow funding for the continued delivery of countywide 0-5 CTS program services for the period of July 1, 2023 through June 30, 2028.
