RECOMMENDATION(S)
Recommendation
1. Approve and accept Amendment No. 1 to Contract No. 26-177 from the United States Department of Health and Human Services, Health Resources and Service Administration, for the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant, increasing the amount by $7,222,313, from $2,791,588 to $10,013,901, with no changes to the period of March 1, 2026 through February 28, 2027.
2. Approve and authorize the submission of the 2027 Non-Competing Continuation Progress Report to the United States Department of Health and Human Services, Health Resources and Services Administration, to receive continued funding from the Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant, in the amount of $10,514,596, for the period of March 1, 2027 through February 29, 2028.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The Ryan White HIV/AIDS Program Part A HIV Emergency Relief Project Grant (Ryan White Part A) is federally funded by the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA). Acceptance of Amendment No. 1 to Contract No. 26-177 will increase the current Ryan White Part A grant award by $7,222,313, from $2,791,588 to $10,013,901, for the period of March 1, 2026 through February 28, 2027.
Approval to submit the 2027 Non-Competing Continuation Progress Report (NCC Report) will allow the Department of Public Health (DPH) to request up to $10,514,596 in continued Ryan White Part A funding from HRSA for the period of March 1, 2027 through February 29, 2028.
Ryan White Part A funding is cost reimbursable and does not require a local match. Adequate appropriation and revenue are included in DPH's 2026-27 budget and will be included in the 2027-28 recommended budget.
BACKGROUND INFORMATION
HRSA provides Ryan White Part A funds for assistance in developing and enhancing access to a comprehensive continuum of high quality, community-based medical care and support services for low-income individuals with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) and their families. DPH will use the funds to provide medical care and support services, including mental health care, case management, and housing assistance, to approximately 3,448 people living with HIV/AIDS in San Bernardino and Riverside counties. These services include medical care, medical case management, mental health services, dental care, substance abuse counseling services, early intervention services, medical nutrition therapy, transportation, housing and food assistance.
The Ryan White Program was enacted in 1990 to provide federal funding for primary healthcare and support services for persons living with HIV/AIDS. Ryan White Part A supports areas disproportionately impacted by HIV, with the goal of improving health outcomes for low-income, uninsured, and underserved individuals.
DPH serves as the lead agency for San Bernardino and Riverside Counties, with both counties first receiving funding in 1994. DPH administers the grant funds, including the development and submission of the annual grant application, acceptance of the grant award, procurement of service contracts, and assurance of compliance with federal grant requirements.
Ryan White Part A is funded annually with a three-year performance period. Year one requires a competitive application, followed by NCC Reports for years two and three.
On September 24, 2024 (Item No. 55), the Board of Supervisors (Board) approved and authorized the submission of a grant application for Ryan White Part A funding, in the amount of $9,460,867, for the period of March 1, 2025, through February 29, 2028.
On March 11, 2025 (Item No. 35), the Board approved and accepted Agreement No. 25-146 from HRSA for Ryan White Part A funding in the amount of $1,662,168, for the period of March 1, 2025, through February 28, 2026.
On August 5, 2025 (Item No. 60), the Board approved Amendment No. 1 to Agreement No. 25-146, increasing the Ryan White Part A funding amount by $2,444,965, from $1,662,168 to $4,107,133, for the period of March 1, 2025, through February 28, 2026.
On September 23, 2025 (Item No. 66), the Board approved the submission of the 2026 NCC Report to continue receiving Ryan White Part A funding for the period of March 1, 2026 through February 28, 2027.
On October 7, 2025 (Item No. 29), the Board approved Amendment No. 2 to Agreement No. 25-146 increasing the Ryan White Part A funding amount by $5,176,533, from $4,107,133 to $9,283,666, for the period of March 1, 2025, through February 28, 2026.
On March 24, 2026 (Item No. 37), the Board approved and accepted Contract No. 26-177 from HRSA, for the Ryan Part A funding, in the amount of $2,791,588, for the period of March 1, 2026, through February 28, 2027.
On July 24, 2026, DPH received an amended grant award to Contract No. 26-177 from HRSA for the Ryan White Part A funding, increasing the amount by $7,222,313, from $2,791,588 to $10,013,901, with no change to the period of March 1, 2026, through February 28, 2027. Recommendation No. 1 will allow DPH to use these grants funds to continue to provide primary healthcare and support services to people living with HIV/AIDS.
Approval of Recommendation No. 2 will authorize DPH to submit the 2027 NCC Report to HRSA, enabling continuation of Ryan White Part A funding for the third year of the grant performance period. DPH will return to the Board to request acceptance of additional Ryan White Part A funding, if and when received. In 2026, HRSA extended the baseline cap of funding for 2027-28 Ryan White Part A funding that DPH could request. Funding was increased by $829,366, from $9,685,230 to $10,514,596.
These items are being presented at this time as this is the first available Board meeting available following the appropriate operational, fiscal, and legal reviews. Approval at this time will not impact DPH’s Ryan White Program services or activities.
