Recommendation
Approve Amendment No. 2 to the following contracts, for the provision of Prevention and Early Intervention Older Adult Community Services program, updating the standard contract language, increasing the combined amount by $1,050,000, from $3,850,000 to $4,900,000, and extending the contract period for an additional one year and six months, for a total contract period of January 1, 2021 through December 31, 2027:
1. Contract No. 20-1138 with Rim Family Services, Inc., increasing the amount by $300,000, from $1,100,000 to $1,400,000.
2. Contract No. 20-1139 with West End Family Counseling Service, increasing the amount by $750,000, from $2,750,000 to $3,500,000.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The recommended increase totaling $1,050,000 for the Prevention and Early Intervention Older Adult Community Services (PEI), is funded by Behavioral Health Services Act (BHSA), Behavioral Health Services and Supports funds. Adequate appropriation and revenue have been included in the Department of Behavioral Health’s (DBH) 2025-26 and 2026-27 budgets and will be included in the 2027-28 recommended budget.
BACKGROUND INFORMATION
DBH provides mental health and substance use disorder (SUD) services to county residents experiencing severe mental illness and/or SUD. The PEI program (Program) promotes healthy aging and supports mental wellness among older adults throughout the County by providing specialized PEI services. PEI services are available to adults aged 60 and older who may be experiencing the onset of a serious mental health condition or who are at increased risk due to factors associated with aging, trauma, and/or bereavement.
The Program includes four components: the Older Adult Mobile Resource Unit, the Older Adult Wellness Services, the Older Adult Home Safety Program, and the Older Adult Suicide Prevention Program. The Program provides short-term early intervention counseling services and linkage to additional resources when appropriate. PEI services are delivered throughout the county in a culturally and linguistically responsive manner to ensure accessibility for diverse older adult populations. The Program is designed to benefit older adults who encounter barriers in accessing PEI mental health services, are at risk of suicide, or are unserved, underserved, or inappropriately served. PEI services include mental health assessments, counseling services, substance use screenings, transportation assistance to medical appointments, and assistance with maintaining appropriate home and personal safety. Since 2009, DBH, with approval from the California Mental Health Services Oversight and Accountability Commission, has provided PEI services to eligible older adult county residents through a Memorandum of Understanding with the County Department of Aging and Adult Services and partnerships with contracted service providers.
On November 17, 2020 (Item No. 30), as a result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 20-1138 with Rim Family Services (RIM), in the amount of $900,000, and Contract No. 20-1139 with West End Family Services, Inc. (West End), in the amount of $2,250,000, for the provision of PEI for the period of January 1, 2021 through June 30, 2025.
On June 10, 2025 (Item No. 38), the Board approved Amendment No. 1 to Contract No. 20-1138 with RIM, increasing the contract amount by $200,000, from $900,000 to $1,100,000, and Amendment No. 1 to Contract No. 20-1139 with West End reflecting their legal name change to West End Family Counseling Service and increasing the contract amount by $500,000 from $2,250,000 to $2,750,000, as well as updating standard contract language, and extending the contract terms by an additional year, for a total contract period of January 1, 2021 through June 30, 2026.
In 2024-25, PEI provided services to:
• 11,170 unduplicated clients
• At an average cost of $63 per client
• Delivered a total of 9,154 services across various regions of the county
Approval of the recommended amendment will ensure the continued provision of PEI services for an additional one year and six months and serve:
• Approximately 8,952 unduplicated clients
• At an estimated cost of $117 per client
Performance measures for the MSFS contracts focus on tracking service quality, timeliness, accessibility, and outcomes to ensure providers meet established standards. Contractors are required to submit regular reporting such as monthly service logs, outcome data, fiscal reports, and compliance documentation, which allows the department to monitor progress and identify trends or concerns. Accountability mechanisms, including performance audits, contract monitoring meetings, site visits, and corrective action plans, help ensure that services are delivered as intended and align with program goals. Together, these structures promote transparency, support continuous improvement, and ensure that community members receive high-quality, effective behavioral health services.
The recommended contract amendments for the provision of PEI services update contract language to ensure integrated compliance with BHSA requirements and to support the transition from Mental Health Services Act (MHSA) funding to BHSA funding. BHSA was approved through Proposition 1 in March of 2024, replacing and updating the former MHSA. BHSA modernizes funding and expands statewide access to behavioral health and SUD services, effective July 1, 2026. BHSA strengthens equity, accountability, and transparency, increases the behavioral health workforce, and expands housing and treatment resources. Additionally, the recommended contract amendments increase funding and extend the term through December 31, 2027, to ensure continuity of PEI services to the community while DBH evaluates a formal procurement amid the BHSA transition.
DBH will continue regularly review 1) client service data and progress, conduct site visits and annual monitoring to ensure performance and compliance standards of the contract(s) are met; and 2) providers’ invoices administratively and programmatically to ensure client and/or bed counts are accurate prior to payment processing.
