Recommendation
1. Approve Amendment No. 20 to Hospital Per Diem Agreement No. 20-887 with Inland Empire Health Plan, to add reimbursement provisions for Community Health Worker services, effective September 1, 2026, with no change to the agreement term of October 1, 2020 through December 31, 2026.
2. Direct the Clerk of the Board of Supervisors to maintain confidentiality of the Amendment pursuant to Health and Safety Code Section 1457(c)(1).

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Costs associated with providing eligible Community Health Worker (CHW) services will be funded through reimbursement from Inland Empire Health Plan (IEHP), pursuant to Agreement No. 20-887. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2026-27 budget.
BACKGROUND INFORMATION
On September 15, 2020 (Item No. 15), the Board of Supervisors (Board) approved Agreement No. 20-887 (Agreement) with IEHP, to provide reimbursement for medical services rendered by ARMC to IEHP members, for the period of October 1, 2020 through September 30, 2023. Nineteen amendments have since been entered into relating to the Agreement to allow ARMC to receive supplemental payments for Medi-Cal managed care capitation rate range increases, update program eligibility, amend compensation and reimbursement rates, enable participation in various incentive programs, and extend the term of the Agreement through December 31, 2026.
On December 16, 2025 (Item No. 11), the Board approved Amendment No. 19 to the Agreement with IEHP, for reimbursement for medical services rendered to IEHP members, to participate in the Global Quality Pay for Performance Program, for program years beginning on 2026, with no change to the contract term of October 1, 2020 through December 31, 2026.
Approval of Amendment No. 20 (Amendment) to the Agreement will enable ARMC to receive reimbursement from IEHP for CHW services provided to eligible IEHP members and to align with Medi-Cal’s coverage of CHW services. ARMC utilizes CHW’s to assist medical personnel with collecting and ensuring the accuracy of information while also supporting patients with accessing preventive care, managing chronic conditions, and addressing health-related needs. The Amendment adds reimbursement and billing provisions for eligible CHW services and incorporates applicable Department of Health Care Services program requirements, including the collection and tracking of health indicators for state-mandated programs, and Medi-Cal billing policies. The Amendment also includes requirements intended to prevent duplicative billing for services reimbursed through other Medi-Cal benefits.
Pursuant to Health and Safety Code section 1457(c)(1), records that reflect specific terms related to the rates of payment for health care services are not immediately subject to laws related to public records; therefore, the Clerk of the Board is directed to maintain confidentiality of the amendment.
