RECOMMENDATION(S)

Recommendation

1.                     Approve amendments to the following contracts to continue providing home visiting services for improved maternal health and infant/child development, increasing each contract amount by $141,104, from $423,312 to $564,416 each, and extending the contracts for eight months, for the total period of November 19, 2024 through June 30, 2027:

a.                     Amendment No. 2 to Contract No. 24-1146 with Child Care Resource Center, Inc.

b.                     Amendment No. 2 to Contract No. 24-1147 with Reach Out.

2.                     Approve Amendment No. 5 to Contract No. 22-15 with El Sol Neighborhood Educational Center, to continue providing home visiting services for improved maternal health and infant/child development, increasing the total contract amount by $759,842, from $7,073,376 to $7,833,218, and extending the contract for six months, for a total contract period of January 11, 2022 through June 30, 2027.

FINANCIAL IMPACT

Approval of this item will not impact Discretionary General Funding (Net County Cost). The total cost increase for the three California Home Visiting Program (CHVP) service contracts is $1,042,050, which includes $141,104 each for Child Care Resource Center, Inc. and Reach Out, and $759,842 for El Sol Neighborhood Educational Center (El Sol). CHVP services are funded by federal pass-through and state funding from the California Department of Public Health (CDPH). Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2026-27 budget.

 

BACKGROUND INFORMATION

The Maternal, Child and Adolescent Health (MCAH) Division of CDPH allocates funding to local health jurisdictions to administer CHVP with the goals of promoting maternal health and well-being, improving pregnancy outcomes and infant and child development, strengthening parenting skills and family functioning, and cultivating strong communities. The MCAH Division is designed to assist families who are at risk for adverse childhood experiences, which include child maltreatment, domestic violence, substance abuse, and mental illness.

 

CHVP services are family-centered and provided in clients’ homes by home visiting staff. Services include routine screening and assessments of parent-child interactions, child development, and maternal depression; referrals to parent support groups, father involvement programs; behavioral health resources, early childhood education, and family-based intervention support. DPH has provided CHVP services through subcontracts with community-based organizations since 2022, serving approximately 200 families per year.

 

On January 11, 2022 (Item No. 25), as a result of a formal procurement, the Board of Supervisors (Board) approved Contract No. 22-15 with El Sol to provide CHVP services in the amount of $2,380,000 for the period of January 11, 2022 through January 10, 2024.

 

On February 28, 2023 (Item No. 46), the Board approved Amendment No. 1 to Contract No. 22-15 with El Sol, increasing the contract amount by $232,000, from $2,380,000 to $2,612,000, with no change to the contract period of January 11, 2022 through January 10, 2024.

 

On October 24, 2023 (Item No. 46), the Board approved Amendment No. 2 to Contract No. 22-15 with El Sol, increasing the contract amount by $1,422,000 from $2,612,000 to $4,034,000, for a total contract period of January 11, 2022 through January 10, 2025.

 

On November 19, 2024 (Item No. 39), the Board approved Amendment No. 3 to Contract No. 22-15 with El Sol, increasing the contract amount by $1,519,688, from $4,034,000 to $5,553,688, and extending the term of the contract for a total contract period of January 11, 2022 through December 31, 2025. The Board also approved Contract No. 24-1146 with Child Care Resource Center, Inc. and Contract No. 24-1147 with Reach Out, in the amount of $211,656 each, for the period of November 19, 2024 through October 31, 2025, to expand the provision of family-centered home visiting services throughout the county.

 

On October 21, 2025 (Item No. 39), the Board approved Amendment No. 4 to Contract No. 22-15 with El Sol, increasing the total contract amount by $1,519,688, from $5,553,688 to $7,073,376, and extending the contract for one additional year, for a total contract period of January 11, 2022 through December 31, 2026. The Board also approved Amendment No. 1 to Contract No. 24-1146 with Child Care Resource Center, Inc. and Amendment No. 1 to Contract No. 24-1147 with Reach Out, increasing each contract by $211,656, from $211,656 to $423,312, and extending the contracts for one additional year, for the total period of November 19, 2024 through October 31, 2026.

 

DPH recommends approval of the recommended amendments to ensure continuity of CHVP services for children and families while the department prepares to issue a request for proposals in fall/winter 2026. DPH monitors the contractors’ performance through reviews of monthly reports and annual site visits to ensure compliance with the administrative, program, and fiscal terms and conditions of the contracts.
