FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The cost of $35,000 is funded by State Medi-Cal, Federal Medicare, private insurance, and other departmental revenues. Funding sources may change in the future pending any legislative activity related to the repeal and/or replacement of the Affordable Care Act. Adequate appropriation and revenue have been included in the Arrowhead Regional Medical Center (ARMC) 2026-27 budget and will be included in future recommended budgets.
BACKGROUND INFORMATION
This Amendment with Q-Centrix, LLC (Q-Centrix), requests an additional $35,000 expenditure to provide clinical data abstraction and coding to support the registry of a backlog of cancer cases. This will assist with backlog remediation from November 2025 to present, bringing registry records into compliance with applicable standards. The Amendment estimates a cost of $20,935 to remediate the backlog, but notes that ARMC will be billed based on actual cost at the rates listed in the Amendment. As a result, ARMC is requesting $35,000 in funding for the Amendment to cover potential overages and prevent any delay that may increase the existing backlog. Abstracting involves extracting required clinical information from medical records, while coding converts that information into standardized classifications used for reporting, quality measurement, research, and regulatory purposes.
The cancer registry backlog resulted from the loss of two Certified Tumor Registrars (CTRs), including the passing of one staff member and the recent resignation of another. Because cancer case abstraction requires specialized expertise and must be performed by qualified, certified registrars, these staffing shortages significantly reduced the department’s capacity to process cases in a timely manner.
If not addressed promptly, the backlog could jeopardize compliance with cancer accreditation standards and reporting requirements. Continued delays may place ARMC’s cancer program accreditation and related certifications at risk, potentially affecting the program’s ability to maintain established quality standards and regulatory compliance. Timely intervention and additional cancer registry support are necessary to reduce the backlog, restore operational capacity, and safeguard ARMC’s cancer program accreditation and related certifications.
The cancer registry services provided by Q-Centrix maintain standardized information about cancer diagnoses and treatments. Q-Centrix will assign the backlog cases to dedicated clinical data specialists to perform case finding, chart review, data abstraction, coding, validation, and completion of registry forms, taking the data through validation and submission to the applicable registry. Through its registry solution, Q-Centrix provides ARMC with visibility into cases, workflow, validation, completeness, and submission status. Their process also involves validation and re-abstraction/reliability checks, including governance and auditability of the resulting data.
On January 17, 2024, as the result of a non-competitive procurement, Purchasing executed the Master Service Agreement (Contract No. 4400024152) with Q-Centrix for data abstraction services for heart procedures (Cath Percutaneous Coronary Intervention) in the amount of $900,000, for the five-year term of February 14, 2024 through February 13, 2029.
On June 9, 2026 (Item No. 8), the Board of Supervisors approved Amendment No. 1 to Contract No. 26-428 (4400024152) with Q-Centrix, including non-standard terms, for the addition of the oncology cancer registry service solutions, for abstracting and coding cancer cases according to standards set by American College of Surgeons and National Cancer Registrars Association, increasing the contract amount by $376,180, from $900,000 to a total contract amount of $1,276,180, with no change to the current term of February 14, 2024 through February 13, 2029.
ARMC recommends the approval of this Amendment to support long term sustainability to meet current and future cancer reporting and programmatic demands in a well-planned and balanced manner.
