Approve Amendment No. 2 to Agreement No. 26-285, from the United States Department of Health and Human Services, Health Resources and Services Administration, for the Ending the HIV Epidemic: A Plan for America - Ryan White HIV/AIDS Program Parts A and B, authorizing the carryover of $1,287,310 in unobligated 2025-26 funding to increase the total approved budget, from $2,592,027 to $3,879,337, with no change to the period of March 1, 2026 through February 28, 2027.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Amendment No. 2 to Agreement No. 26-285 authorizes the carryover of $1,287,310 in prior-year funding from 2025-26 from the United States Department of Health and Human Services, Health Resources and Services Administration (HRSA) and does not require a local match. Adequate appropriation and revenue have been included in the Department of Public Health’s (DPH) 2026-27 budget.
BACKGROUND INFORMATION
The Ryan White Program was enacted in 1990 to provide federal funding for comprehensive primary healthcare and support services for persons living with human immunodeficiency virus/ acquired immunodeficiency syndrome (HIV/AIDS). DPH, acting as the lead agency for the San Bernardino and Riverside counties, develops and submits the grant application, accepts the grant award, administers the funding, procures service contracts, and ensures compliance with federal grant requirements.
The Ending the HIV Epidemic: A Plan for America - Ryan White HIV/AIDS Program Parts A and B (EHE) grant funds efforts to reduce new HIV infections throughout the United States. The funding is used to provide support services and interventions, through subcontractors, which link persons living with HIV to care and treatment facilities throughout San Bernardino and Riverside counties. Services include outpatient medical care, case management, linkage/reengagement to care, food, and transportation services, as appropriate. Approximately 1,983 unduplicated clients are served through EHE each year. This item accepts carryover funds from the 2025-26 grant budget period that were unspent, allowing DPH to use these funds in the current grant period to enhance and expand services and activities.
On October 8, 2024 (Item No. 28), the Board of Supervisors (Board) approved the submission of a grant application in the amount of $25,000,000, for the period of March 1, 2025 through February 28, 2030.
On February 25, 2025 (Item No. 29), the Board approved Agreement No. 25-122, in the amount of $1,076,917 for the period March 1, 2025 through February 28, 2026.
On August 19, 2025 (Item No. 45), the Board approved Amendments Nos. 1 and 2 to Agreement No. 25-122, increasing the amount by $481,335 and $2,162,788, respectively, for a total amount of $3,721,040, for the period of March 1, 2025 through February 28, 2026.
On October 7, 2025, (Item No. 31), the Board approved Amendment No. 3 to Agreement No. 25-122, increasing the funding by $873,003, from $3,721,040 to $4,594,043, for the period of March 1, 2025 through February 28, 2026.
On March 3, 2026, DPH received notification of a partial grant award (Grant Award No. 5 UT8HA33958-0700), in the amount of $668,595, for the period of March 1, 2026 through February 28, 2027.
On April 21, 2026 (Item No. 33) the Board approved Agreement No. 26-285 (Grant Award No. 5 UT8HA33958-0700), in the amount of $668,595, for the period of March 1, 2026 through February 28, 2027.
On August 4, 2026 (Item No. 66), the Board approved Amendment No. 1 to Agreement No. 26-285, increasing the funding by $1,923,432, from $668,595 to $2,592,027, for the period of March 1, 2026 through February 28, 2027.
On July 15, 2026, DPH received Amendment No. 2 to Agreement No 26-285, which authorizes the carryover of funding in the amount of $1,287,310, increasing the total approved budget for the period of March 1, 2026 through February 28, 2027 from $2,592,027 to $3,879,337, with no change to the federal funds obligated for this budget period.
This item is being presented at this time, as this is the first available date following the required operational, fiscal, and legal reviews. Approval at this time will not impact DPH’s Ryan White Program services or activities.
