RECOMMENDATION(S)
Recommendation
1. Approve and accept grant awards from the California Department of Public Health for the following 2025-26 grant funding, in the aggregate amount of $6,339,414.01, for the period of July 1, 2025 through June 30, 2026:
a. Maternal, Child, and Adolescent Health program (State Agreement No. 202536), in the amount of $646,742.02.
b. California Home Visiting Program (State Agreement No. CHVP SGF EBHV 25-36), in the amount of $2,279,944.
c. Black Infant Health program (State Agreement No. 202536), in the amount of $3,412,727.99.
2. Approve the purchase and distribution of prepaid restrictive gift cards to provide essential support and incentives to eligible Black Infant Health participants, in the purchase amount not-to-exceed $3,200, for the period of June 23, 2026 until fully distributed.
3. Authorize the Chair of the Board of Supervisors, Chief Executive Officer, or the Director of the Department of Public Health to execute and submit any subsequent non-substantive amendments and documents necessary, to receive the grant funding, such as the scope of work or budget, on behalf of the County, subject to review by County Counsel.
4. Direct the Director of the Department of Public Health to transmit all non-substantive amendments to the agreements listed in Recommendation No. 1 to the Clerk of the Board of Supervisors within 30 days of execution.

FINANCIAL IMPACT
Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). The grant funding from the California Department of Public Health (CDPH), in the amount of $2,279,944 for the California Home Visiting Program (CHVP) does not require a match. The funding for the Maternal, Child, and Adolescent Health (MCAH), in the amount of $646,742.02, and the Black Infant Health (BIH) program, in the amount of $3,412,727.99, consist of federal Title V, federal Title XIX funds, and State General funds. The MCAH program requires a local match of $264,087.28, and the BIH program includes a local match of $645,745, which will be met with existing ongoing 1991 Realignment funds.
The purchase amount for the prepaid gift cards will not exceed a total of $3,200 and will be funded by State General Funds through the BIH program. Adequate appropriation and revenue are included in the Department of Public Health’s (DPH) 2025-26 recommended budget.
BACKGROUND INFORMATION
CDPH allocates funding to local health jurisdictions for the MCAH, CHVP, and BIH programs (collectively, programs) with the goals of promoting healthy living for mothers and their families by addressing comprehensive health, physical, and psychosocial needs, with a concentration on reducing infant mortality, enhancing maternal and child well-being, and improving pregnancy outcomes, infant and child development, parenting skills, and family functioning to cultivate strong communities.
DPH contracts with several community-based organizations to provide services under these three programs.
MCAH program staff collaborate with healthcare providers, community-based agencies, and community groups to identify health issues that affect women, infants and children, including those with special healthcare needs. This collaboration develops best practices and interventions intended to increase access to care for mothers and families, especially those economically disadvantaged with limited access to services.
CHVP serves pregnant and newly parenting families who are at risk of domestic violence, unstable income or housing, limited education, substance use disorders, among others. Services include screening and assessments to determine families at risk, routine assessment of parent-child interactions, child development and maternal depression, referrals to parent support groups and father involvement programs, behavioral health resources, early childhood education, and family-based intervention support.
BIH centers on meeting the health needs of pregnant and postpartum Black and African American women through an evidence-informed model built on group intervention, social support, and case management. Activities include weekly group sessions, one-on-one support, assistance with enrollment into supportive social services, and connecting participants to appropriate levels of care to improve pregnancy outcomes and reduce maternal and infant morbidity and mortality.
In addition, the BIH program uses state funds to offer restrictive gift cards, transportation, childcare, food and other essential motivators. These supports strengthen engagement in BIH activities and improve access to health-related services. Restricting gift cards to specific vendors or categories ensures funds are used appropriately and in alignment with BIH goals.
On September 2, 2025, pursuant to County Policy 05-13, the County Administration Office (CAO) authorized the submission of the 2025-26 Agreement Funding Applications (AFA) for the following programs: CHVP in the amount of $2,279,944, MCAH in the amount of $646,742.02, and BIH in the amount of $3,452,716.06, for the period of July 1, 2025 through June 30, 2026.
On May 21, 2026, DPH received approval notification of the MCAH AFA from the CDPH, in the amount of $646,742.02, for the period of July 1, 2025 through June 30, 2026.
On June 2, 2026, DPH received approval notification of the CHVP AFA from CDPH, in the amount of $2,279,944, for the period of July 1, 2025 through June 30, 2026.
On June 4, 2026, DPH received approval notification of the BIH AFA from CDPH, in the amount of $3,412,727.99, for the period of July 1, 2025 through June 30, 2026.
Since the CAO’s approval of the CHVP, MCAH, and BIH AFA on September 2, 2025, the application documents have been under CDPH’s review. CDPH advised that DPH will be reimbursed for all program activity expenditures incurred during the grant period for all three programs.
DPH is recommending delegated authority to accept and approve any non-substantive amendments in relation to the CHVP, MCAH, and BIH grant funding agreements, including adjustments to the scope of work or budget reallocations, provided that the overall approved amount and term does not change. Any substantive amendments to these grants will be brought to the Board for approval.
The grant awards are being presented at this time, as this is the first available date following CDPH’s completed review of the grant application documents and the required operational, fiscal, legal reviews. Approval at this time will not impact DPH’s services or activities.
