RECOMMENDATION(S)

Recommendation

1.                     Approve Amendment No. 1 to Agreement No. 25-413 with the Inland Empire Health Plan, a Medi-Cal Managed Care Plan, extending the term by four months, for a total agreement period of May 20, 2025 through November 1, 2026, with no change to the amount of $300,000.

2.                     Approve Amendment No. 1 to Funding Agreements with the following Medi-Cal Managed Care Plans, extending the term an additional year, for a total agreement period of May 20, 2025 through June 30, 2027, with the option to extend the term two successive one-year periods:

a.                     Kaiser Permanente, Agreement No. 25-414, increasing the amount by $135,905 from $215,000 to $350,905

b.                      Molina Healthcare, Agreement No. 25-415, increasing the amount by $94,817, from $150,000 to $244,817

3.                     Approve Employment Contract template for the following positions, to staff and support community engagement of Community Health Improvement Plan events, under the Medi-Cal Managed Care Plans, effective upon execution through June 30, 2027, with the option to extend the term for a maximum of two successive one-year periods:

a.                     Health Educator Specialist II

b.                     Health Educator Specialist I

c.                     Office Specialist

4.                     Authorize the Director of the Department of Public Health to complete and execute individual Employment Contracts on behalf of the County, including non-substantive changes, utilizing the Employment Contract template identified in Recommendation No. 3, effective upon the date of full execution through June 30, 2027, and to execute amendments to extend the term of the individual Employment Contracts for a maximum of two successive on-year periods, subject to County Counsel review.

FINANCIAL IMPACT

Approval of this item will not result in the use of Discretionary General Funding (Net County Cost). Service deliverables and positions hired under the employment contract template will be fully funded by Medi-Cal Managed Care Plans (MCPs) directly to the Department of Public Health (DPH). Upon acceptance by the Board of Supervisors (Board), DPH will receive increased awards from the MCPs in the amounts of $135,905 from Kaiser Permanente (Kaiser) and $94,817 from Molina Healthcare (Molina). Adequate appropriation and revenue have been included in DPH’s 2025-26 and 2026-27 budgets.

 

BACKGROUND INFORMATION

As part of the statewide California Advancing and Innovating Medi-Cal (CalAIM) initiative, the California Department of Healthcare Services (DHCS) requires all MCPs throughout California to collaborate with local health jurisdictions in the development and implementation of their Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The CHA and CHIP are required for DPH to maintain national accreditation with the Public Health Accreditation Board. DHCS requires MCPs to contribute resources in the form of direct funding, in-kind staffing and data to promote a comprehensive understanding of the multiple factors contributing to community health outcomes. MCPs have agreed to provide funding to DPH , subject to the availability of funding.

 

The funding from each MCP will allow DPH to increase foundational capabilities by hiring two Health Educator Specialist IIs, three Health Educator Specialist Is, and one Office Specialists to staff a variety of community engagement events and ensure quality improvement during the CHIP implementation process. MCP funding will also cover costs of a short-term consultant contract, selected through competitive procurement, to conduct stakeholder training and CHIP implementation evaluation and recommendations. Investment in these areas will drive lasting advances in public health capacity and systems to better serve and support those throughout the county.

 

Approval of the amended agreement with Inland Empire Health Plan (IEHP) will extend the term through November 1, 2026, with no change to the amount of $300,000. Approval of the amended agreements with each of the other MCPs will increase funding by $135,905 from Kaiser and $94,817 from Molina, for a combined total of $230,722. These agreements will be for a total period of May 20, 2025 through June 30, 2027, with the option to extend the term two successive one-year periods. Approval of this item will ensure continuity of activities and maintain the partnership between DPH and each MCP to meet DHCS requirements and support the effective development and implementation of the CHA and CHIP.

 

Recommendation No. 3 requests approval of an Employment Contract template to hire staff responsible for providing administrative support and participating in community engagement events that advance CHIP strategies and activities, effective upon execution through June 30, 2027, with the option to extend the term for a maximum of two successive one-year periods.

 

Recommendation No. 4 authorizes the Director of DPH to complete and execute individual Employment Contracts, including non-substantive changes, and to exercise the option to extend the term of the individual Employment Contracts for up to two successive one-year periods, on behalf of the County, subject to County Counsel review.
